Advanced Nurse Practitioner

Job summary

We are looking for an ANP/CP to join our nursing team. The role is part-time with hours to be negotiated.

To ensure the delivery of an effective triage service by acting as an autonomous practitioner working independently and in conjunction with other health care professionals to assess, diagnose, and treat the conditions of patients within primary care initiating direct referrals as appropriate.

Provide professional advice that is evidence and research based, to patients, carers and colleagues and to ensure the maintenance of clinical excellence.

Follow national and local clinical guidelines and governance principles.

To provide education and training to other staff and students as needed.

Main duties of the job

Working as an independent practitioner preferably within a primary care setting or similar. Management of patients with long term conditions.

Minimum 1 year + experience at a senior level.

At least 2 years primary and community nursing experience (or similar).

Nurse-led management of minor illness, minor ailments and injuries.

Nurse-led triage.

Compiling protocols and clinical guidelines.

Accountability of own role and other roles in a nurse-led service.

Clinical governance issues in primary care.

Wider health economy

We are a 5 partner dispensing Practice located inthe heart of the market town of Pershore with around 10,600 patients in Pershore and the surrounding villages. Our aim: to providehigh quality modern medical care; to enable easy access to a full range ofservices, in an attractive environment and atmosphere.

We have 9 GPs, 1 GP Registrar, 2 Advanced Nurse Practitioners, 5 Specialist PracticeNurses, 2 Practice Nurses, 1 Clinical Pharmacist, 1 Pharmacy Technician, 1 Healthcare Assistant and 1 Nurse Associate.

Staff - a great team of helpful professionals including ANPs who triage same day urgent appointments, practice nurses whose duties include the management of chronic disease and clinical pharmacists who deal with all medication queries, hospital discharge letters and assist with GP workload.

Building - we have a modern, spacious building with staff parking. We have excellent clinical facilities including a minor surgery suite. We also have a large bright and comfy staff room where staff meet daily for networking and coffee.

Job description

Job responsibilities

Undertake Clinical Nursing Practice at an advanced level to deliver holistic care to people accessing primary health care services. Assess, diagnose and treat patients contacting primary health care services with a range of acute and chronic medical conditions. Prescribe medicines and appliances as determined by patient need. Directly admit patients to secondary care hospital in acute medical need. Refer patients to other medical specialities or to other members of the multidisciplinary team for assessment if deemed appropriate. Interpret diagnostic tests and routine clinical procedures. Advise and support others as needed. Work within professional guidelines and codes of conduct, acknowledging own limitations. Work within all relevant practice policies and procedural guidelines, infection control. Chaperoning, risk management. Independent Nurse prescriber. Contribute to the practice targets QOF, ReVivo. Contribute to disease registers. Contribute to vaccination campaigns.

Experience

Essential
