Main duties of the job
You will help identify patients with palliative and end of life care needs who would benefit from care services, improve timely support for patients and families, work collaboratively with other health care professionals and improve communication within the community will act as an advocate for patients and carers, signposting to appropriate services as well as providing direct care
You will be working alongside our Hospice Registered Nurses, Hospice Support Workers, Pre and Post Bereavement Family Support Practitioners and Complementary Therapist to offer wraparound care to patients with palliative and end of life care needs and their carers.

Hours: 24 hours perweek. Flexibility offered in start and finishtimes and days of week
Salary: £23,899 plus travel costs of 45p per mile
JobPurpose: Toenable palliative and end of life patients to access services and support in atimely manner, including practical, emotional and clinical care

OverallAim:
The role is to inform and enable patients who have palliative andend of life needs and are currently on the GP palliative care register andtheir carers to access appropriate services. The post holder will work incollaboration with a defined group of GP practices within West Northumberland todevelop and integrate services to provide timely access for patients withpalliative and end of life needs. This will promote earlier referral tocommunity and hospice services, which will reduce inappropriate admissions andlengthy stays in acute settings.
MainDuties and Responsibilities
Manage own caseload of patients workingclosely with community teams and Tynedale Hospice at Home Care co-ordinators tosupport consistency of care given. Attend and contribute to regular MDT meetingswithin GP practices and wider based meetings when appropriate to end of life. Identify and communicate with patients on GP palliativecare register who are not accessing care services who would benefit from input. Deliver education and training for staff indelivery of evidence-based end of life care. Assess, plan, implement and evaluate nursingcare provision to meet individual patient needs. Provide end of life and palliative care trainingand expertise within the defined group of GP practices in West Northumberland. Undertake nurse prescribing functions withinthe defined scope of practice. Audit effectiveness of end of life andpalliative care nursing interventions. Demonstrate continuous service improvement,through learning from patient feedback and incidents. Work collaboratively with the wider team topromote the earlier introduction of wraparound Hospice services. Promote and develop the use of wraparoundservices within current wellbeing hubs. Undertake the role of Practice Supervisor/Assessor to nursing and medical students. Contribute to THH strategy, developingservices to patients with palliative and end of life needs and their families,auditing hospice services to identify areas of development and improvement. Contribute to collection and reporting ofappropriate Key Performance Indicators for palliative and end of life care.Key Contacts:
