An excitingopportunityhas arisen for a 12 monthsecondment into the North Primary Care Liaison Service.

We would welcome applications from a current band 4, or experienced band 3 with relevant community experience. The role would be to support the clinical team.

This role will afford rapid access to credible specialist mental health expertise for GPs, Primary Care staff and other referring agents who are requiring support and expert advice on meeting the mental health needs of the people within their case loads. This will ensure that patients in Primary Care have their mental health needs more clearly assessed, defined, cared for and treated by Primary Care Health Care Professionals and that the pathways to best meet someone's needs is optimised. For most people this will mean remaining within Primary Care supported by the Primary Care Liaison Service where needed, but for some, needs necessitating, there will be a smooth transfer into AWP treatment services.

Main duties of the job

This role is focused on supporting people, in the community, with a range of acute mental health needs, who may be in crisis, and their carers (including friends and relatives) and supporters, enabling and assisting them to meet daily health, social care and well being needs, in line with personal recovery goals, and facilitating engagement with mainstream services. The post holder will contribute to the ongoing assessment, planning, delivery and review of activities and interventions against identified health, social care and well being needs, acting as care/recovery coordinator for an allocated group of service users.

You will be well supported by the Band 6's within the team, alongside the Team manager and Senior Practitioner with regular supervision.

Job description

Job responsibilities

1. Promote and champion the rights, values, responsibilities and diversity of people.

2. Promote and maintain effective communication with people in order to build and sustain positive working relationships with individuals, families and carers, team members and other agencies. This may include accessing translation and interpreting services.

3. Contribute to the planning, implementation and review of care packages, including implementing parts of an individual care plan.

4. Work autonomously within relevant practice boundaries, under indirect, but not direct, supervision of registered clinicians.

5. Conduct comprehensive assessments of health needs and clinical risks.

6. Maintain accurate, confidential records of patient activity, contributing to a range of written documents as appropriate, including reports, referrals and letters.

7. Enable people to understand and manage their health needs in line with their care plan and relapse prevention strategies. This may include enabling people to manage their own domestic and personal resources, housing needs, and enabling people to access learning / training, or work opportunities.

8. Support individuals in developing and maintaining their identity and personal relationships, including potentially difficult relationships.

9. Support people with health and wellbeing needs to engage with their own health care, in accordance with their care plan.

10. Act to protect individuals from abuse and harm in line with local and National policies and procedures ( Safeguarding, Risk Management, Suicide Prevention).

11. Work proactively with carers and families, identifying carers needs and signposting to appropriate services or agencies.

12. Proactively develop your knowledge and practice, making use of both clinical and management supervision and available learning opportunities.

13. Promote, monitor and maintain health, safety and security in the workplace.

Person Specification

Education & Qualification

Essential

If not held, willingness to complete NHS Care Certificate If not held, willingness to complete NVQ Level 5 in Mental Health etc

Desirable

Experience of working as a healthcare support worker in a clinical team. An understanding of how mental health issues can affect the lives of individuals, their families and their community. Knowledge of, and experience of working with, a range of mental health issues

Ability to communicate clearly in both written and verbal form about routine matters Ability to use computer systems efficiently and effectively to record data clearly and accurately. Ability to conduct oneself in a professional and tactful manner, and to champion core care values such as dignity, compassion and safety.
