Job summary
Weare recruiting to the post of Discharge Coordinator on an Endocrine ward. Thepost holder will be responsible for the arranging of safe discharges from theward. We are a friendly team and are always keen to welcome new members to theward.
Main duties of the job
Therole of the successful candidate will be to arrange discharges of the patients.This will entail communication with patients, relatives and the multidisciplinaryteam to implement the services needed for safe discharge of our patients.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Job responsibilities
Arranging discharges for thepatients.
Discussion with patients andrelatives about what support is needed in the discharge process.
Discussion with multidisciplinaryteam members including social services, the discharge management team anddistrict nurses.
Discussion with ward physiotherapistsand occupational therapist.
Liasing with discharge lounge to arrangetransport home.
