Main duties of the job

The post holder will be a qualified Health Visitor . You will work autonomously as part of the Health Visiting Team managing a designated caseload of children and families, in order to improve their health and enable them to reach their full potential.

You will manage an identified enhanced caseload and work collaboratively with other agencies to provide high quality care to children and families. Child protection work will involve gathering and analysing information to make informed assessments, appropriate referrals and participating and contributing at case conferences and core group meetings.

To identify and monitor the health needs of a given population and facilitate the implementation of evidence based health care.

To develop public health initiatives in line with local and national priorities which promote, protect and improve the health of the population and reduce health inequalities.

To support the Team Lead/Manager CYP in developing innovative practice

Duties and Responsibilities:

To assess, plan, implement and evaluate the provision of care for children and families in accordance with the principles and practice of Health Visiting by undertaking home visits and contacts at other community venues e.g. Children Centres

To manage and prioritise workload according to the identified health needs of the children and families within the community ensuring the effective use of resources. To lead on the delivery of the Healthy Child Programme (2009). To be responsible for the supervision, line management and delegation of duties of to the skill mix team Delivery of key performance indicators (Kpis) within required time scales To compile caseload/community profiles/health needs assessments in order to establish the health needs of the community and to plan and implement appropriate public health initiatives. To review regularly with the Team Leader and Child Protection Supervisor in supervision families causing concern, record agreed action and follow through according to Trust and Safeguarding policies. To liaise with the School Health Service, with particular reference to vulnerable children, their families and children with special needs. To adopt a team approach to Health Visiting, working in close collaboration with other members of the team, offering support and providing reciprocal cover for colleagues in their absence To organise, develop and participate in public health activities including health promotion with individuals and groups. To adopt champion roles in line with the 6 High Impact Areas of the Healthy Child programme and other specialist roles To develop effective partnerships and relationships with primary and secondary health care colleagues, statutory and voluntary agencies. To work within the policies and guidelines set out by the Trust and NMC Code of Conduct. To take responsibility for Nurse Prescribing in line with National and local guidelines. To inform the Team Leader/ Clinical Lead about work load constraints that may adversely affect the level of service which can be provided. To ensure that all untoward incidents, accidents and areas of clinical risk are identified and reported in accordance with Trust policies and procedures. To keep contemporaneous notes and accurate records and maintain client confidentiality according to NMC and Trust guidelines. To undertake such other duties as may be reasonably required from time to time as are consistent with the responsibilities of the post. To promote, develop and foster an environment that is conducive to learning for students and skill mix staff including mentorship and preceptorship of junior staff and students. To participate in research/audit and statistical data which are approved and agreed. To contribute to policies and the development and implementation of care pathways. To advise on the treatment of minor ailments/illnesses in children. To promote the uptake of immunisations for all children in accordance with Department of Health guidelines. To practise as a Nurse Prescriber in accordance with the NMC and Organisational procedures and guidelines, providing appropriate advice and treatment for children and families and reviewing effectiveness of any medication prescribed. Administration of BCG immunisations if required/undertaking new born blood spot screening for under ones

Communication

Dealing with sensitive or contentious information To participate in identified meetings such as case conferences, core group meetings, strategy meetings and other meetings as appropriate. To participate in multi-agency working and develop links with Childrens Social care, school nursing and local GP surgeries in order to facilitate effective information sharing. To assist in developing and maintaining good working relationships with all colleagues, children and families To be conversant with the Trusts Children/Adult Safeguarding Procedures, referring any concerns as appropriate. Policy Development To participate and contribute to research, audit and evaluation of new ways of working.
