Job description
Job responsibilities
To prepareassessments, intervention/review reports and discharge To provide highlydeveloped specialist assessments of CYP referred to the PALMS Team based uponappropriate use, interpretation and integration of complex data from a varietyof sources including semi-structured interviews with CYP, family members andothers involved in the CYPs care, self-reports measures, rating scales anddirect & indirect structured observations
To formulate andimplement plans for the formal intervention and/or management of CYP presentingdifficulties, including challenging behaviours, toileting, feeding, sleepingconcerns, problematic sexualized behaviour, and/or mental health difficulties;based upon an appropriate conceptual framework of the CYPs needs and employingmethods based upon evidence of efficacy, across the full range of care settingsand with other colleagues and services as appropriate
To be responsible fora complex caseload whilst working alongside a multi-disciplinary team toprovide a coordinated and integrated individualised care package to childrenand young people.
