The Planned Care Team is responsible for transforming health services in our area. In line with the reforms outlined in the Health and Care Act 2022. This role will ensure that services meet people's needs effectively, improving care and health, and take responsibility for spending NHS money wisely.
We are recruiting a Planned Care Manager to support with leading on improving outcomes for people in Hertfordshire and West Essex, with a particular focus on the South and West Hertfordshire Place. The role will support delivery of key, high profile, commissioning projects. This will include supporting management of NHS contracts, supporting tender exercises, public consultations and strategy development.
You will work with partners across the Health and Care Partnership (HCP) including NHS, County Councils, Voluntary Community Faith and Social Enterprises (VCFSE) and others to deliver innovative and flexible solutions.

Main duties of the job
Supporting the business in driving transformation as well as value for money. Support with strategic and operational management of elements of the planned care programme function including planning and overseeing health services to develop strong service delivery. Support collaboration and partnership working to integrate services for patients, driving local pathway design and implementation through the ICB and the Health and Care Partnership (HCP) governance structures, supporting co-ordination of Place based development and leadership, with a particular focus on developing strategic and strong working relationships across the South and West Hertfordshire (SWH) Place. Work in close partnership with wider strategic partners including community and clinical leads across the ICB and the HCP, support with a number of high-profile areas of transformation/redesign to deliver the strategic and clinical priorities agreed by the Integrated Care Partnership. This includes supporting with significant redesign under the planned care programme, ensuring integrated care and improved population health is delivered by high quality services, closer to home for patients through integration across health and social care, providing better value for money.

Knowledge

Educated to masters level or equivalent level of experience of working at a senior level in specialist area. Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to master's level equivalent Evidence of post qualifying and continuing professional development
