Main duties of the job

To support the integrated 0-19 Healthy Child Programme (HCP) in providing an innovative and high quality service in an environment that enhances child centred client care and promotes child and family wellbeing.

To support the assessment and review of clinical needs.

To work without direct supervision to support clients as part of the universal and universal plus aspects of the service provision.

To liaise with colleagues within the clinical team to assist in the co-ordination and development of the service.

To develop specialist interest areas in order to enrich the knowledge and skills set within the Luton Children & Young Peoples Service.

There is also an expectation that you will rotate across 0-19 service, to ensure that all relevant clinical skills are kept up to date, and to maintain effective working relationships.

Job responsibilities

Main Duties and Responsibilities

To hold own caseload with support from Specialist Community Public Health Nurse (SCPHN) for those families with planned packages of care (Specialist and Targeted intervention). Assist in the delivery and planning of programmes/activities either with individual parents/carers or groups of parents/carers which aim to improve outcomes for children and young people. Assist in the planning and delivery of programmes and activities to meet the service specific targets identified, such as; breastfeeding promotion, smoking cessation, obesity/healthy eating, oral health, speech and language, family health promotion activities, school readiness, developmental assessments and health & wellbeing needs assessments for school aged children. Work in partnership with parents/carers, families and multi-disciplinary colleagues to maximise service delivery and to maintain effective relationships and communication. Support families in accessing advice and guidance regarding childcare and child-health related information. To assist the clinical team in the administrative management of referrals, sharing information with and from health and social care staff. Support to families with more complex needs under the guidance and instruction of senior staff. Support safe and effective information sharing as required for Child Protection case conferences and core group meetings adhering to the Local Safeguarding Childrens Board Policies and Procedures. To write clinical records in a concise, accurate and timely manner in line with Cambridgeshire Community Services record keeping policy. Safely maintain confidential, professional records which are accurate, relevant, up to date and legible. Child Protection/Early Help Assessment Provide direct support and guidance to identified families in their homes or community locations and the promotion of their health and wellbeing Work in collaboration with the SCPHN Health Visitor/School Nurse and partner agencies Local Authority Early Years Alliance, family Hubs ,Childrens Centres, Flying Start and Early Help to provide a safe and effective service to children and their families.

Communication

Create, maintain and enhance effective working relationships within the team, with other staff, and other agencies. Promote multiagency working to provide best care for families and young people. Keep accurate and contemporaneous records. Deliver high quality, evidenced based information to families, individually and in group scenarios. Communicate professionally, demonstrating active listening and the ability to ask sensitive and appropriate questions with excellent verbal and non-verbal communication skills.

People Management

Responsible for supporting junior and skill mix staff as delegated by leadership team Support with Development of junior and skill mix staff as required Maintaining own professional development and requirement to take part in appraisal and KSF process

Clinical and Practice Governance

Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act Any data that is taken/shared as part of a phone call or transported, faxed, or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies. The post holder must adhere to the Trust risk assessment and risk management processes The post holder must adhere to infection control policies and procedures It is a condition of your employment that you are currently registered with NMC and it is your responsibility to maintain your professional registration. Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust. The post holder must participate in clinical and other audits as required Provide patients and their families /carers with information on standards they should expect from the team Participate in Clinical & Safeguarding supervision on a regular basis The post holder is required to participate in relevant emergency preparedness process for their team.

Qualifications and Training

Essential

Registered Nursing Associate on the NMC Register GCSE Maths and English Grade 5 and above Nursing Associate foundation degree qualification

Children and young people related qualifications

Skills and knowledge

Essential

Understand the scope of the role of the Nursing Associate in context of the nursing and interdisciplinary team and the organisation; including how the role may contribute to service development. Ability to organise and prioritise own delegated workload. Ability to communicate effectively, written, verbal, or non-verbal communication, with patients/relatives, carers and all members of the multi-disciplinary team. Ability to support, supervise, assess and act as a role model to nursing associate students, other learners and support workers as required within the clinical setting. Ability to safeguard and promote the welfare of children and young people/vulnerable adults Knowledge of IT packages such as Word, Outlook or SystmOne

Desirable

Record keeping & report writing. Knowledge of equality and diversity issues Awareness of the role of the health visitor and school nurse

Experience

Essential

Previous work within a community setting Experience of working in partnership with other relevant services CAMHS, specialist perinatal mental health, IAPT and 3rd sector organisations.
