St John's Medical Centre is a large, fast paced GP surgery located in Grantham, offering excellent transport links with easy assess to the A1 and on the mainline for London. Two notable Grammar schools in Grantham are Kesteven and Grantham Girls School and The Kings School. We currently have circa 14,000 patients registered at the Surgery and retained our 'Good' rating in our latest CQC Regulatory Review.

We are flexible with regards to the number of hours for the roles but would be looking for hours per week, working Monday- Friday between the hours of 08:00am-18:30pm.

Main duties of the job

Working as an Independent Practitioner, the successful candidate will be required to assess, diagnose and manage patients with a variety of conditions. We offer a same day service and their maybe a requirement for occasional home visits to be undertaken.

The main duties of the role include but are not limited to:

Being able to conduct a comprehensive physical health assessment of patients with complex multiple healthcare needs. Interpret results of multiple different assessments and investigations to make a diagnosis, and plan to deliver care. Prescribe and work with patients to manage their medicines, and to be able to make onward referrals where required. Competently make ethical, evidenced decisions and interventions when faced with complexity and assess the risk associated with these decisions Comfortable working both independently but also as part of a multi-disciplinary team. Comfortable with IT, our clinical systems include SystmOne, Accurx and AskmyGP. (Full training and support of these systems can be provided). To work as Duty Buddy on selected days to help the Duty Doctor with the management of tasks, admin and results.

St John's Medical Centre has a friendly and welcoming team. In addition, we are a Training Practice and regularly welcome GP Registers and Medical Students. Currently we have 3 GP Registrars and a Student Nurse on placement with us.

Our Clinical Team consists of 3 Partners, 2 Salaried GP's, 2 Clinical Pharmacists, 3 Practice Nurses, 2 Health Care Assistants, a Diabetes Nurse and our Home Visiting Team. Amongst these team members, there are special interests in Diabetes, Respiratory Disease, Gastroenterology and Sexual Health.

In addition to our clinical team, we have our Reception, Admin and Management Teams that are vital in the daily running of the Practice and supporting our Clinicians delivery of excellent patient care.

Advanced Nurse Practitioner

Primary Responsibilities

Working autonomously in an independent and Advanced Nursing role Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients Implement and evaluate individual specialised treatment plans for chronic disease patients Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF Undertake the collection of pathological specimens Process and interpret pathology and other test results as required Maintain accurate clinical records in conjunction with extant legislation using primary care software Ensure read codes are used effectively Maintain chronic disease registers Prioritise health issues and intervene appropriately Support the team in dealing with clinical emergencies Recognise, assess and refer patients presenting with mental health needs Extended and supplementary prescriber, adhering to extant guidance Support patients in the use of their prescribed medicines, reviewing annually as required Contribute to practice targets (QOF etc.), complying with local and regional guidance Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.) Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual) Support the clinical team with all safeguarding matters, in accordance with local and national policies Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately Deliver opportunistic health promotion where appropriate Participate in the administrative and professional responsibilities of the Practice Team Ensure accurate and legible notes of all consultations and treatments are recorded in the patients care record Ensure accurate completion of all necessary documentation associated with patient healthcare and registration of the practice Ensure collection and maintenance of statistical information required for regular adhoc reports and audits Ensuring when significant events are identified, they are recorded appropriately on GP Team Net.
