Job summary

To be responsible for theimplementation of processes for the effective management of patients withlong-term conditions using evidence-based practice.

Working as a member of the practicemultidisciplinary team, the post holder will ensure nursing services aredelivered effectively to the entitled patient population.

The Practice Nurse will be responsiblefor a number of clinical areas such as infection preventionand control, health promotion, chronic diseasemanagement, health promotion, well women and well man clinics, as well asactively supporting the practice management team in the reviewing and deliveryof clinical policy and procedure.

About us

Borth Surgery is situated in the centre of the village and has transport links via bus and train. The Surgery looks after approximately 2,700 patients in Borth and the wider community. We have three GP's, one GP Registrar, Physician's Associate all of whom are supported by the Nursing team and admin/reception staff. We are a teaching practice and have a number of students throughout the year.

Our philosophy at Borth surgery is to provideeffective, quality patient care in a range of areas. This includes not onlycaring for patients physical needs but also providing patients with healthpromotion and education regarding their chronic conditions, empowering them tomake decisions and changes that will benefit their health and well being.
