This is a part time position of 24 hours worked over four days
Main duties of the job
The main duties and responsibilities are:
To organise and co-ordinate the provision of nursing services for the practice in liaison with other members of the nursing team and other Primary Health Care professionals
Provide nursing treatments to patients in participation with General practitioners or independently, to agreed guidelines and patient group directives
Provide general and specific health screenings to the patients with referral to general practitioners as necessary both in the practice and for our patients who are either housebound or living in residential and nursing homes attached to the practice

Craneswater Group practice is a two practice organisation, just a stones throw from Southsea seafront. We support 11,500 patients
The team comprises of six GP partners two salaried GPs, three nurse practitioners, two practice nurses and three HCAs. We also have three managers and a team of 40 staff.
Established training practice with successful applicant having the opportunity to develop this role. Good CQC rating in all areas and high QOF achievement and consistently one of the highest rated practices overall for patient experience in the National GP patient survey. Craneswater Group Practice is a member of Portsmouth South Coast Network with shared pharmacist, physiotherapists and social prescriber. JOB TITLE Practice Nurse
MAIN PURPOSE OF THE POSTTo assist medical personnel in the care of ourpatient population to include treatment, preventative care, screening andpatient education in the practice and in the local community.
RESPONSIBLE TO:-
The Partners for clinical matters.
The Practice Manager for administrative matters
MAJORDUTIES AND RESPONSIBILITIESProvidingassessment, screening treatment services and health education advice.

Pathologicalspecimens and investigatory procedures1)Undertakethe collection of pathology specimens, bloods. swabs, smears etc., for dailycollection at mid-day.
2)Performany investigatory procedures requested by the doctors.
Administrative andprofessional responsibilitiesToparticipate in the administration and professional responsibilities of thepractice team.
1)Ensureaccurate details of all treatments (where applicable) are recorded in patientsnotes.
2)Ensureaccurate data entries are made on the computer record of any patient contactand admin issues, including consultations, telephone advice and clinical facts, blood pressures, peak flows, cervical status, vaccinations &immunisations.
3)Ensureaccurate completion and submission of all administrative forms which needforwarding to the Business Manager, Overtime sheets, holiday requestslips, study day requests etc
4)Ensurecollection and maintenance of statistical information required for regular andad hoc reports.
5)Attendand participate in practice meetings as required.
6)Maintaina notice board in waiting areas designated for patient health care/education asrequired.
7)Ensurethat any information relating to personal details is not discussed or divulgedto any unauthorised person and is regarded as strictly confidential at alltimes.
