Job summary

We are looking for an experienced Practice Nurse to provide an immunisations, cervical smear tests and chronic disease management service for our patients. Mondays to Fridays only, but must be prepared to work until PM on Tuesdays evenings (later start possible to compensate).

Modern purpose-built surgery in a quiet residential location. Small friendly team with plenty of interaction with colleagues. Parking available on a first-come first-served basis, but close to Mill Hill East Underground station and bus services. Waitrose and Virgin Active Gym close by.

Package includes , NHS Pension contributions, sick pay plan, weekly clinical team meetings.

About us

We are a small friendly team located in a modern purpose-built building with some parking available, but with free parking on surrounding streets also. Mill Hill East tube (Northern Line) is a few minutes walk as are bus services. Waitrose and a Virgin Active Gym are close by.

We have a full-time partner, three part-time salaried doctors and two physician associates, an advanced care practitioner, a healthcare assistant and nurses' administrator, a referrals secretary, practice manager, reception manager and a small team of reception staff, two part-time reception administrators and two patient document administrators. We also have a team of Primary Care Network (PCN) staff including social prescribers, care coordinators, first contact physiotherapists, pharmacists, a health and well-being coach and a mental health practitioner.

We open Mon-Fri, AM - PM with extended hours on Tuesdays until PM. We offer appointments in the evenings and weekends via our PCN hub surgeries and Barnet Federation.

We hold weekly clinical teammeetings and monthly practice team meetings.

We use EMIS Web clinical system and Docman. We operate a telephone triage system using AccuRx for same day clinician appointments. Nurse appointments are 15 minutes long.

You will assist in maintaining up-to-date PGDs, infection prevention and control, any relevant logs, QOF, immunisation and smears targets.

Must be flexible, good-natured and polite, punctual with good written and spoken English.

PRACTICE NURSE

Accountable to the Doctors.

Hours: Negotiable but must include extended hours currently on Tuesday evenings until 20:00

Job Summary :

Working in accordance with the UKCC Code of Conduct and with reference to Practice Guidelines, appropriately referring to GPs, the post-holder will provide assessment, treatment, screening, health education services and advice to patients.

The post-holder will work with reference to Practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.

The post-holder will have the required level of training and competence and will work within patient group directions where these are available. A nurse with prescribing qualifications will use them appropriately.

Basic Skills, Duties and Responsibilities Needed :

You require at least the following:

Immunisation (Adult and Child)

Administering appropriate immunisation autonomously Working with patients with more complex immunisation issues.

Travel Health

Administering injections and providing guidance in accordance with guidelines Identifying problems Provision of specialist and evidence-based support to the team.

Blood pressure recording

Be able to take blood pressure and record the results accurately

Urinanalysis

Be able to test samples and/or pass on samples for hospital checking, as required

ECG

Be able to record an ECG and load it onto computer for assessment by the doctors

Assessment and Advice

Give dietetic advice and information Give information about Asthma re: PEFR and devices.

Computer Skills

Be able to use a computer (EMIS Web), Docman 10, as necessary (though training can be given). Maintain accurate documentation of care and utilisation of

Room Care

Maintain the treatment/nurses room and all instruments at a very high standard of cleanliness for the purposes set out above.

Stock Monitoring

Assist the HCA in monitoring stock of

Sample containers (blood, urine, faeces) Vaccines Syringes and needles Swabs and dressings Couch rolls and other nursing supplies Drugs stock for GPs

Identifying significant abnormalities Diagnosis, monitoring and development of individual management plans; agreeing these as appropriate with the patient and other health professionals

Therapeutic Monitoring

Checking compliance with and adherence to appropriate treatments using an holistic patient-centred approach Recognising abnormalities Identifying the impact of treatment and implementing or recommending changes as appropriate.

Wound Care and Management

Assessing routine wounds including trauma Assessing [and prescribing] to support good wound management in complex situations ( use of Doppler technique)

Patient Health Checks

Identifying significant abnormalities Obtaining appropriate information using a lifestyle questioning approach and identifying appropriate health promotion issues Working with patients to develop a management plan where health problems or potential health problems are identified

Risk Assessment

Recognising issues and gathering sufficient information to refer ( drugs; domesticviolence; child protection; vulnerable adults; senior patients; social problems)

Health Screening

Recognising issues and gathering sufficient information to refer ( womens and mens health, sexual health, older people) Undertaking monitoring tasks ( smears) and providing advice as appropriate Providing in depth monitoring and advice as appropriate to the level of the post-holders specialty and working with the patient in deciding on management plans. Working independently or alongside the general practitioner and making decisions/ recommendations as appropriate.

Smoking Cessation

Be able to advise, refer and manage patients wishing to stop smoking. Submit claims for smoking cessation work done.

DES/LES/LCS etc

Be willing to act as lead on relevant commissioned and enhanced services, sending out invitation letters, and submitting claims, as may be needed.

Other

Dealing with more complex problems and prescribing if appropriate.

Meetings/In-house Training Sessions:

You are required to attend:

Nurse/HCA Team meetings held every other Monday at pm Clinical meetings, held every Tuesday at PM Staff meetings, which are held on the second Thursday of each month from Monthly training sessions, which are held on the fourth Thursday of each month from till approximately (if applicable).

Over-time :

Over-time (which will be remunerated at the usual hourly rate) may be worked, as and when required, with prior agreement by a partner or the practice manager.

Study Leave :

You are allowed five days study leave pro rata each year (April to March) and agreement for you to attend is at the discretion of the Partners/Practice Manager. Any additional study leave requested will be granted at the Partners discretion.

Annual Leave :

The post attracts 25 days' annual leave plus Bank and Statutory Holidays per annum, April to March each year.

Experience

Essential

Experience of working in accordance with the UKCC Code of Professional Conduct and The Scope of Professional Practice. Practice Nursing qualification and experience needed. Experience of EMIS Web clinical system preferred. Experience of Microsoft Office software. Experience of dealing with the public/patients. Good communication skills. Pleasant and articulate.

Desirable
