Job summary

We are recruiting an Advanced Clinical Practitioner to joinour multi-disciplinary clinical team providing a high standard of care for ourpractice population of 4,500 patients.

Our team are friendly, approachable and support each other well ensuring a pleasant working environment.

We are in a modern purpose built building, have free staffparking and are close to good public transport links.

Main duties of the job

The ACP role involves telephone consultations, face to faceconsultations and a daily home visit as required.

The successful applicant will demonstrate critical thinkingskills to make safe clinical decisions, following practice policies andprocedures. They will work collaboratively with practice colleagues to providea high standard of care and meet the needs of patients.

About us

Our clinical team is led by 2 GP partners and comprises ofGP's, ACP's PracticeNurse, Healthcare Assistants and are all supported byour reception team.

We have a close practice team who arefriendly, approachable and supportive. We strive to provide a high standard ofcare for our patients with the resources available to us and working inpartnership with wider health and social care services.

The practice is an active member of Spen Health & Wellbeing PCN, working with other local practices to develop population care.

Job description

Job responsibilities

The following are the core responsibilities of the advancedpractitioner. There may be on occasion, a requirement to carry out other tasks;this will be dependent upon factors such as workload and staffing levels:

a. Manage patients presenting with a range of acute andchronic medical conditions, providing subject matter expert advice to patientsin practice or in their homes for housebound patients

b. Implement and evaluate individual specialised treatmentplans for chronic disease patients

c. Identify, manage and support patients as risk ofdeveloping long-term conditions, preventing adverse effects on the patientshealth

d. Provide advanced, specialist care to patients as requiredin accordance with clinical based evidence, NICE and the NSF

e. Request pathology services as necessary

f. Process and interpret pathology and other test results asrequired

g. Maintain accurate clinical records in conjunction withextant legislation

h. Ensure read codes are used effectively

i. Maintain chronic disease registers

j. Chaperone patients where necessary

k. Prioritise health issues and intervene appropriately

l. Support the team in dealing with clinical emergencies

m. Recognise, assess and refer patients presenting withmental health needs

n. Extended and supplementary prescriber, adhering to extantguidance

o. Support patients in the use of their prescribed medicinesor over the counter medicines (within own scope of practice), reviewingannually as required

p. Contribute to practice targets (QOF etc.), complying withlocal and regional guidance

q. Liaise with external services / agencies to ensure thepatient is supported appropriately (vulnerable patients etc.)

r. Delegate clinical responsibilities appropriately(ensuring safe practice and the task is within the scope of practice of theindividual)

s. Support the clinical team with all safeguarding matters,in accordance with local and national policies

t. Understand practice and local policies for substanceabuse and addictive behaviour, referring patients appropriately

u. Deliver opportunistic health promotion where appropriate
