Job summary

The service is for adults who have learning disabilities and / or autism who are either:

Being discharged from hospital with a forensic history and active risk profile Living in the community with a forensic history and active risk profile Living in the community and are at risk of engaging in offending behaviours

The service will cover the geographic areas of Essex, Southend and Thurrock. The objectives for the service are to:

Reduce the offending and reoffending rates of people with learning disabilities Prevent inappropriate admissions to inpatient services, where risk is indicated, but no mental ill health Increase access to specialist advice supporting positive practice with service users, rather than containment and restrictive practice Increase the capacity and capability of community providers to work with people with a forensic history or where risks are identified

Main duties of the job

Learning Disabilities and / or Autism who either have or are at risk of engaging in offending behaviours.

To take an active part in the development of LD & Forensics Care pathways. To take responsibility as a member of the Essex Community Forensic Service for the updating of the CQC record of Compliance. To adopt a Customer focus & business minded approach to service delivery. To participate in local and SBU wide policy & procedure reviews & development. To actively lead on audit initiatives for Essex Community Forensic Service. At all times, to actively ensure that people with learning disabilities are included in and have access to service development and improvement planning. To positively promote learning disability services and Inpatient Assessment & Treatment services as specialist services for people with learning disabilities in Essex. To take part in local forums and engagement events so as to gather and collate feedback from the general public and make ensuing recommendations for service development.

Job responsibilities

To work in close liaison with forensic colleagues and learning disability professionals in delivering effective evidence-based practice to adults with To carry out assessment of individual needs, make recommendations of how to meet those needs, support the implementation of these recommendations and monitor, evaluate and review the input. To act in a consultative role to external agencies in relation to clinical interventions and risk assessments & management plans. To maintain appropriate records according to NMC Record Keeping & HPFT Data Quality policy and guidance. To support the ongoing transforming care agenda. To promote and maintain good working relationships and effective communications / liaison with carers, team colleagues in North and South Essex learning disability services, the legal system and external agents. To ensure the Essex Community Forensic Service works within the Mental Health Act (1983, amended 2007) and Essex Safeguarding Adults frameworks. To provide clinical supervision and leadership to Community Nurses working in the Essex Community Forensic Service To encourage and motivate others to work flexibly to meet the needs of the service. Understanding and adhering to local, regional and national policies, procedures and guidelines, ensuring innovative and safe practices. To participate where appropriate in the development and implementation of policies, procedures, standards and guidelines in response to relevant new legislation. Person Specification

Qualifications

Essential

RNLD or RMN ENB 997/998 (Mentoring & Preceptorship) or equivalent Post graduate qualification

Desirable

NVQ Level D32/33 (Assessors qualification)

SKILLS AND KNOWLEDGE

Essential

Post registration experience Experience of working with the criminal justice system Knowledge of health related practice, policy and legal requirements. Experience in care coordinating hospital discharges for detained patients. Management of junior staff and ensuring service priorities are met. Proven skills in clinical and managerial supervision. Experience & skills in management of stressful situations ensuring recognition and support to staff and service users Teaching and presentation skills. Audit as a part of performance review and improving clinical practice. Advanced knowledge & skills in Risk Assessment & Risk Management. Experience in completing structured clinical judgement risk assessments, eg HCR20. Ability to lead on Nursing Assessment's, Devise, Implement and Evaluate Care Plans.

Possess excellent communication skills in a variety of mediums, spoken, written & electronic. Highly developed skills in use of non-verbal communication.
