Job summary
An exciting new opportunity has arisen to work as an Assistant Practitioner AHP Band 4 alongside the Occupational Therapy and Physiotherapy with the Community Multiple Sclerosis Team (CMST) and Regional Disability Team (RDT). CMST and RDT provide community based neuro rehabilitation for people 16 years and over who have a neurological condition (including Multiple sclerosis ( Services are delivered throughout the region both within Walkergate Park in a gym setting and in the community).
CMST and RDT are well-established teams and are passionate about delivering neurological rehabilitation and achieving best outcomes. This exciting opportunity will offer the successful candidate to be part of these services, as well as opportunities to develop themselves within the area of neurological rehabilitation in the community setting.
Main duties of the job
This is a full time post working under the direction of the qualified Therapists. You will assist the Therapists to assess and deliver therapy and activity within the objectives of the treatment plan. In addition there will be opportunities to develop skills through the provision of group therapy sessions as well as individually-tailored therapy sessions to service users.
You will require excellent communication, time management and organisational skills and be able to work autonomously as well as part of a team.
You will be supported by experienced Therapists receiving supervision and CPD opportunities.

CMST and RDT are multidisciplinary teams working in partnership with numerous agencies, including secondary care and third sector organisations. Job responsibilities
Assist the therapist to plan, design and develop therapy and intervention activities within the objectives of the treatment plan.
Use specialist technical and creative skills to facilitate the preparation andimplementation of AHP occupation/communication focused tools and approaches as a member of the MDT in order to promote recovery, wellbeing and maximise independence.Deliver 1:1 and group treatment to patients monitoring their progress and making minor adaptations as required within sessions.
Contribute their observations of the communication and occupational barriers and strengths of the individuals based on a foundation theoretical knowledge of communication, language and occupation.

Education & Qualification
Essential
Foundation degree in applied social studies or alternative (NVQ/BTEC/QCF) with additional training to diploma level equivalent in the speciality field. Demonstrates knowledge and skills applicable to the post
