This opportunity is to work for two practices in Alpha PCN. Gossoms End Surgery in Berkhamsted and the Rothschild House Group in Tring. The working pattern is open to discussion but likely to be three long days at Gossoms End, Berkhamsted and one long day at Rothschild House Group, Tring.

F irst round of interviews will take place via Teams Meeting within two weeks of closing date.

Direct patient contact through assessment, examination,investigation, diagnosis and treatment

Firstpoint of contact of care to establish a working diagnosis and management planin partnership with the patient

Main duties of the job

A Post Graduate Degree in PhysicianAssociate Studies from an accredited PA Programme

Membership of the Faculty ofPhysician Associates at the RCP, which includes UK Physician AssociateRegistration (the PA Managed Voluntary Register)

Current and valid certification orre-certification by the UK Physician Associate National Examination

Experience of working in a medicineenvironment, including preventative medicine and chronic disease management

Flexible and adaptable approach to avaried and high-volume workload with the ability to multi-task and to workunder pressure

Consistently compassionate andrespectful approach placing a high value on the dignity of patients, relatives,carers and colleagues.

Ability to communicate effectivelywith colleagues, patients, relatives, nurses and other staff and agencies

Primary Care Networks (PCNs), supported by Dacorum Healthcare Providers Limited(DHPL), are developing new and exciting additional roles under the 2020 scheme,to support GPs and clinical teams at enhancing patient services in the localcommunity.

Job responsibilities

ResponsibleFor:

Theresponsibilities of the role include direct patient contact through assessment,examination, investigation, diagnosis and treatment. You will maintain a key role in supportingdelivery of network contracted services and to provide a holistic clinicalservice, with support from GPs as required.

JobPurpose:

To helpsupport practices with structured care around patients with long term conditions,including frailty.

Providefirst point of contact of care for patients presenting with undifferentiated,undiagnosed problems utilising history-taking, physical examinations andclinical decision-

makingskills to establish a working diagnosis and management plan in partnership withthe patient (and their carers, where applicable).

Will havedirect and immediate access when needed to a GP/Doctor and work under supervision of the GP with which the patientis registered.

Review,analyse and action diagnostic test results.

Deliverintegrated patient centred-care through appropriate working with the widerprimary care multi-disciplinary team and social care networks.

Undertakeface-to-face, telephone and online consultations for emergency or routineproblems, as determined by the PCN, including management of patients withlong-term conditions. Where required by the PCN, offer specialised clinicsfollowing appropriate training including (but not limited to) family planning,baby checks, COPD, asthma, diabetes and anticoagulation.

Undertakehome visits and participate in duty rotas.

Must possess a valid driving licence.

Providehealth/disease promotion and prevention advice to patients.

Utiliseclinical guidelines and promote evidence-based practice and partake in clinicalaudits, significant event reviews and other research and analysis tasks.

Throughparticipating in continuing professional development opportunities, keep up todate with evidence-based knowledge and competence in all aspects of the role,meeting clinical governance guidelines for continuing professional development(CPD).

Develop andagree a personal development plan (PDP) utilising a reflective approach topractice, operating under appropriate clinical supervision.
