Job summary

We are looking for a driven and caring person to join our Admin team at the Dove River Practice in a Care Co-Ordinator role. You will work within our friendly team to support our Clinicians and provide excellent care to our patient population. You need to be motivated and compassionate, someone who can forward think and problem solve, to help us to provide the best possible care to our patients.

The role is for 25 hours a week, over 5 days.

Main duties of the job

You will work to ensure that our patients have the best Health Care journey possible. Helping them and the system to improve and administer tracking processes along the patient pathway to ensure the patients journey is managed efficiently, smoothly and in accordance with targets and agreed timescales.

You will use various IT systems including our GP IT system EMIS and will be confident in engaging with all other providers. You will also work in partnership with the acute clinical teams, to ensure the patient journey is seamless and as efficient as it possibly can be. Your role will also be to remove obstacles and duplication to ensure a consistent and effective administration service is always maintained.

About us

The Dove River Practice is a two site GP practice providing care for around 8500 patients from our surrounding villages. We have Surgeries in both Sudbury and Tutbury with GPs and nursing teams at both.

The Practice has two GP Partners, three Salaried GPs and a supporting clinical team of Nurses, HCAs and other associated roles.

Job description

Job responsibilities

1.Utilise population health intelligence from EMISand other dashboards to proactively identify and work with cohorts of patientsto deliver personalised care, including those who are frail, housebound, orwith diagnosis of Learning Disabilities or other health issues as required.

2.Support patients to utilise decision aids inpreparation for a shareddecision-making conversation regarding theircare.

3.Holistically bring together all of a personsidentified care and supportneeds, and explore options to meet thesewithin a single personalisedcare and support plan (PCSP), in line withPCSP best practice, based onwhat matters to the patient.

4.Help patients to manage their needs throughanswering queries, makingand managing appointments, and ensuring thatpeople have good qualitywritten or verbal information to help them makechoices about their care.

5. MaintainDenosumab and Prostap/Nebido/Zoladex registers. Ensure all patients arecorrectly recalled and meds ordered in line with Practice procedures, andshared care agreements are received and in place where required.

6.Assist people to access self-management educationcourses, social prescribing, peer supportor interventions that supportthem in their health and wellbeing andincrease their activation level.

7.Explore and assist people to access personal healthbudgets whereappropriate.

8.Provide coordination and navigation for people andtheir carers acrosshealth and care services, working closely with socialprescribing linkworkers, health and wellbeing coaches, and other primarycareprofessionals.

9.Provide scanning support to the Practice over two mornings a week asstandard (more may be required for sickness/holiday cover)

10.Clinical coding of records/clinical letters into patient health records,and arranging follow up as required to streamline patient care.

11.Support the coordination and delivery of MDTswithin the PCN.

Key wider responsibilities

You will work with the GPs and other primarycare professionals within the Practice to identify and manage a caseloadof patients, and where required and as appropriate, refer people back toother health professionals within the GP practice or associatedteams. You will be responsible for raising awareness within the Practice of shared decision making and how to identify patients who may benefit from this. You will assist patients in accessing Primary Care Services, including liaising with the IM&T Manager to ensure strategies are in place to ensure the Practice achieves National Targets set for the uptake of online services and ensure that all medical information gathered by the Practice is utilised effectively in the care of our patients.

You will also have a knowledge of basic safe-guarding processes in place for vulnerable individuals; and provide opportunities for the patient to develop friendships and a sense of belonging, as well as build knowledge, skills and confidence.

Full training from internal teams and external providers will be provided to enable you to achieve this.

Person Specification

Skills and Abilities

Essential

Excellent communication skills. Excellent interpersonal skills. Good personal organisation. Proficient in computer skills, including knowledge of Microsoft Word and Excel. Ability to follow procedures. Ability to work unsupervised and to supervise others when necessary.

Qualifications

Essential

Evidence of a good standard of literacy and numeracy (Maths and English Grade C).

Desirable

Knowledge and understanding of medical terminology

Experience

Experience working in an office environment. Practical experience of computerised recording systems.

Desirable

Experience of EMIS clinical system including search and reporting. Experience working with DOCMAN and workflow of clinical correspondence following procedures and policies. Experience of coding patient contact and basic clinical information into EMIS. Experience of working in General Practice or other Primary/Secondary care environment.
