Are you an enthusiastic, passionate, forward thinking Occupational Therapist looking to develop their skills and experience? The Community Learning Disability in Ferndown and Purbeck are looking for you to join

You will be able to demonstrate your commitment to work proactively and in partnership with people with learning disabilities, their families and carers, supporting them to live successfully in the community supporting a wide range of needs. You will maintain effective links with mainstream services, working in partnership with agencies to support equal access and promote independence alongside a multi-disciplinary team. You will be committed to improving outcomes for people with a Learning Disability and have excellent interpersonal and communication skills in supporting health and occupational needs.

As the Community Learning Disability Occupational Therapist, you will be joining a workforce who is staffed by a passionate, supportive and dedicated team to improve our patient's quality of life. This is an excellent opportunity to practice Occupational Therapy within the rewarding and diverse field of community learning disability whilst developing your skill set.

TheCommunity Learning Disability Occupational Therapistpost is a part time role of 30 hours. We can be flexible with how these hours are met which can be discussed further in interview. Being a driver is essential for this role.

Main duties of the job

To provide support and work directly with people with learning disabilities in their home, including working with their families and Care & Support providers.

To work with the individual, their carers and members of the multi professional team in the formulation, implementation and review of a person-centred care and support plan. This will include risk assessments and management plans that will outline the support the individual requires to continue to live successfully within the community.

To ensure clear communication pathways are maintained to a high standard with relevant professionals.

To support a wide range of staff who work in social services and the independent sector providing clinical expertise and advice in aspects of behaviour and learning disability raising their awareness to ensure they meet the specific needs of the service user.

To undertake a care management role when it has been identified the majority of care required is health care and it is felt that there is an added benefit top the individual for a nurse to undertake this role. This will include co-ordinating meetings to review the quality and delivery of care.
