The post will be based at Newhaven Community Ward, which is on the site of the old Newhaven Downs Hospital. The unit is for patients who no longer require acute hospital care but need further skilled nursing and therapeutic support such as physiotherapy and occupational therapy before leaving NHS inpatient care altogether. A team of nurses, therapists, doctors and support staff will care for 34 patients in the ward, supporting them to recover before they move on.

We are a stand-alone unit and part of University Hospitals Sussex NHS Foundation Trust. We work closely with the wards and departments at RSCH and PRH.

We welcome applications from those who currently hold a UK NMC registration. If you currently hold an overseas nursing qualification and require sponsorship we are not currently direct hiring Internationally Qualified Nurses.

Main duties of the job

Applicants for this role will have a keen interest in patient rehabilitation and promoting independence, combined with an ability to provide care in a compassionate and dignified manner.

The successful applicant will be numerate and literate, with excellent communication skills.

The ideal candidate would be expected to deliver the highest standards of care to our patients and have a keen interest in falls prevention, dementia care, tissue viability, nutrition and hydration.

You will also have the opportunity to gain experience in complex discharge planning. The post holder needs to work well within a team and be able to work with a wide variety of staff to maintain standards in line with Trust and NMC guidelines.

You will be supported by the senior nursing team. You will be given a local ward induction, a trust induction and access to a robust preceptorship programme aimed at supporting you to develop within the first few months of starting. There is also ongoing support through regular appraisals, competency based learning, and relevant study days.

Continuing education and professional development are strongly encouraged and opportunities will be made available for the successful applicant as we work closely with our Higher Education partners to support nurses reach their full potential. Detailed job description and main responsibilities

Clinical

Is responsible for the assessment, planning and implementation of care and assists in the interpretation of data relevant to the patient’s condition and progress Assists in the identification of patient problems and needs with the patient, their carers, senior nursing staff and other staff as appropriate Identifies the priorities of care for patients Identifies realistic goals for patients in care plans Gives skilled care to patients and provides support to their carers Administers drugs and treatments as prescribed according to the NMC guidelines and the Trust’s Guidelines for the Safe and Secure Handling of Medicines Changes care plans and goals according to evaluation and discussion with the multidisciplinary team Demonstrates an understanding and skill in the care and management of the ill patient Works closely with all members of the care team to ensure the needs of patients are met Demonstrates proficiency in the care of CVP lines and the administration of IV Therapy in accordance with Trust Policy. Participate in the smooth and effective discharge planning for patients maintains accurate documentation to facilitate the continuity of care for patients transferred to other areas Ensures that discharge review documentation is completed and reimbursement material is communicated to relevant body in timely fashion Liase with nursing and clerical staff to enable efficient administration of ward work

Professional 

Demonstrate a professional approach to work in accordance with the NMC Code of Professional Conduct at all times To have knowledge of and adhere to ward and Trust policy and professional standards. To maintain confidentiality, privacy and dignity surrounding the patient/client’s admission and treatment at all times Ensure that documentation is complete and up to date, in line with NMC and Trust guidelines To meet PREP requirements. To identify own learning needs and ensure own professional development is maintained by keeping up to date with practice developments Ensure that mandatory and statutory training needs are met. Registered General Nurse with current NMC registration or soon-to-qualify

Desirable criteria

Mentorship module / or similar/ i.e. NMC Supervisor/Assessor (ENB 998) Evidence of professional development since qualification Knowledge of rehabilitation and the Band 5 role
