Main duties of the job

As a Team Leader you will be responsible for the line management and support of a team of nursing staff. You will be heavily involved in innovating team development as we move into Integrated Neighbourhood Teams, moving the team forward whilst keeping the service user and care we offer as our central focus. You will hold exemplary communication skills, enabling you to be a role model for others, as well as positively representing Mid Yorkshire hospitals NHS teaching trust.

The Integrated Care Team provides excellent nursing care to the residents of The Wakefield District. The service aims to provide a hospital avoidance service and to support individuals returning home for a period of short-term rehabilitation following a serious illness/injury. We provide an OPAT service for patients within their own homes, to prevent long hospital stays. Additionally, the team provides nursing care for end of Life patients and covers the Community nursing service out of hours.

Job responsibilities

The Team Leader will be the clinical expert and professional resource to the Multi-Professional team, patients and carers within the Integrated Care Team.

The post holder will take overall responsibility to develop the Integrated Care team in partnership with the Locality Matron to work collaboratively to develop services and quality of care delivered.

The post holder will manage and lead the whole service team comprising of nurses, nursing associates, coordinators and healthcare support workers and in addition lead the development and delivery of a programme of practice and professional development for staff within the speciality. This will be achieved through working in partnership with lead clinicians, managers and matrons.

Develop a high level of clinical decision making, monitor and improve standards of care, working within their advanced scope of professional practice.

Lead clinical care by managing a patient caseload providing expert assessment, planning and evaluation.

Co-ordinate and manage nurse-led clinics (within Speciality/service).

Contribute towards a multi-professional approach in the management of patients within the specialist area, especially in relation to ongoing care needs and discharge arrangements.

Support patients and their families and ensure that they receive the required information to enable them to self manage, lead or participate in their care delivery.

To support, assist and encourage the multi-disciplinary team in resolving patient problems, by the provision of new and innovative models of case management.

Work with the Matron to promote the development of nurses undertaking the CNO Ten Key Roles as considered appropriate Nurse Prescribing.
