We are lookingfor a motivated Practice Nurse to join our experienced and forward-thinking practiceteam. Civic Medical Centre is a well-established practice and is passionateabout providing a high-quality service to it patients.

Main duties of the job

Tobe responsible for the delivery of practice nursing services, working as partof the practices multidisciplinary team who provide care within their scope ofpractice to the entitled patient population. The practice nurse will beresponsible for a number of clinical areas such as health promotion, chronicdisease management, health prevention, well woman and well man clinics, as wellas supporting the management team in the reviewing of clinical policy andprocedure.

Clinical system is EMIS web.

We are a member practice of Healthy South Wirral PCN who are a very forward thinking, proactive and supportive PCN.Ideally, we are looking for a Practice Nurse with respiratory management experience, however, we are prepared to invest in training for the right candidate who may not have such experience.

Job description

Job responsibilities

The followingare the core responsibilities of the practice nurse. There may be on occasion arequirement to carry out other tasks; this will be dependent on factors such asworkload and staffing levels:

Primary Responsibilities

a.Develop,implement and embed health promotion and well-being programmes

b.Implementand evaluate individual treatment plans for chronic disease patients

c.Identify,manage and support patients at risk of developing long-term conditions, preventingadverse effects on patients health

d.Provideroutine nursing care to patients as required in accordance with clinical-basedevidence, NICE and the NSF

e.Providewound care (ulcer/Doppler etc.) to patients

f.Providetravel medicine services

g.Requestpathology services as necessary

h.Processpathology results as required

i.Providechronic disease clinics, delivering patient care as necessary, referringpatients to secondary/specialist care as required

j.Maintainaccurate clinical records in conjunction with extant legislation

k.Ensureread codes are used effectively

l.Maintainchronic disease registers.

m. Chaperone patients where necessary

n.AssistGPs with minor surgery when required

o.Prioritisehealth issues and intervene appropriately

p.Supportthe team in dealing with clinical emergencies

q.Recognise,assess and refer patients presenting with mental health needs

r.Implementvaccination programmes for adults and children

s.Supportpatients in the use of their prescribed medicines or over-the-counter medicines(within own scope of practice)

t.Liaisewith external services/agencies to ensure the patient is supportedappropriately (vulnerable patients etc.)

u.Delegateclinical responsibilities appropriately (ensuring safe practice and that thetask is within the scope of practice of the individual)

v.Supportthe clinical team with all safeguarding matters, in accordance with local andnational policies

w.Understandpractice and local policies for substance abuse and addictive behaviour,referring patients appropriately

Secondary Responsibilities

a.Actas the audit lead, effectively utilising the audit cycle

b.Monitorand ensure the safe storage, rotation and disposal of medicaments

c.Supportjunior members of the nursing team, providing guidance when necessary

d.Participatein local initiatives to enhance service delivery and patient care

e.Supportand participate in shared learning within the practice

f. Continually review clinical practices,responding to national policies and initiatives where appropriate
