Job summary

Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.

Clinical and Care Professional Leadership (CCPL) roles have been established in SEL ICB since April 2022 and have been a valuable source of clinical leadership.

Within the Community-based Care (CBC) directorate, the role of the CCPL for Rising Risk and Frailty has an important remit, providing advice and support on complex system-wide transformation for patients living with frailty; and those living with increasing complexity who are at risk of exacerbation and hospitalisation and can be proactively supported to stabilise to use more planned than unplanned care services.

Main duties of the job

The immediate priorities for the postholder will be to:

o support the scoping, definition and/or development of a tool to identify our 'risking risk' population,o support the agreed priorities of the CBC directorate, in line with the SEL Joint Forward Plan, as well as the priority areas of proactive care, improving care planning and consolidation of our admission avoidance offer. o act as a clinical champion for the further development of a consistent, personalised model of care, operated by INTs, in line with the key recommendations of the Fuller Report.

The role will strongly link with the other CCPL roles within the CBC directorate as well as Frailty, Proactive Care and Admission avoidance related CCPL roles at place. The postholder will be expected to make links with other relevant stakeholders at Programme, Place, and regional level such as Place frailty clinical leaders, Community providers, Palliative Care, UEC clinical leads, London Frailty Clinical Network, and London Palliative and End of Life Care Clinical Network

In agreement with the postholder, a more detailed workplan will be developed after appointment.

Qualifications and Experience

Essential

Qualified care and/or clinical professional NMC, HCPC, social work register, GMC etc. Educated to Masters level or equivalent level of experience of working at a senior level in specialist area. Experience of transformational/innovative service delivery, productivity and/or transformation policy implementation. Previous experience of being part of peer networks and collaborative working

Desirable

Good knowledge or cross professional transformation, digital solutions and innovative redesign. Cross organisational and/or professional boundary collaboration experience

Knowledge and Skills

Essential
