Job summary

Enthusiastic full-time Pharmacy Technician to join our team, responsible for the safe, accurate and timely supply of prescribed medication to patients, working directly with patients, community pharmacy, local nursing homes and following strict guidance. They will also provide technical and administrative support to Clinicians and support the clinical team with queries regarding medication from patients.

Main duties of the job

Preparation of repeat prescriptions, transcribe medications from discharges and clinical letters, respond to medication queries from patients with the support of Clinicians and Community/Collaborative Pharmacists. Support medication reviews and reconciliation, assist with patient safety audits as required Responsible to Practice Manager and Partners.

Dinas Powys Medical Centre is an established GP practice based in the Vale of Glamorgan. We are a multi-professionl training practice working across Dinas Powys, Llandough and Penarth.

We have a team of 5 GP Partners, 3 Salaried GP's, 3 Nurses, 2 Health Care Assistants all of whom are supported by our admin and reception teams. We offer a range of enhanced services, as well full GMS core services and encourage staff in the development of special areas of interest. We are part of the Eastern Vale Collaborative and work closely with our colleages in our local area.

We are situated just outside Cardiff with excellent road and rail links, in a purpose built medical centre. As a Practice we aim to provide excellent and sustainable care to our patients and community, with everyone in our team supported, valued and appreciated.

Pharmacy Technician Job Description

Job Title: Pharmacy Technician

Reports to: Partners (Clinically)

Key Tasks & Responsibilities

Responsible for repeat prescriptions ensure done in timely manner. Review and re-authorise repeat prescriptions within own competence, in line with practice protocols, arranging any necessary monitoring. Assess the suitability of requests for acute prescriptions within own competence and per practice protocols. Day to day handling of prescription queries within own competence from patients, carers, care homes, pharmacies, hospital teams etc - via telephone or at front desk Identify suitable alternatives for medicines that are unavailable. Manage serious out of stock notices proactively to avoid any delays or breaks in therapy for patients. Medicines reconciliation post discharge and on other transfers of care, ensuring recommendations are within local guidelines/formularies and not conflicting with existing medication. Work closely with Health Board and Cluster Pharmacist. Contribute to the review and development of repeat prescribing protocols to improve the efficiency and effectiveness of repeat prescribing systems. Liaise with other pharmacy stakeholders Medicines Management Tam, community pharmacy and hospital pharmacy team. Liaise with community pharmacies regarding the provision of medicines charts, compliance aids, medication reviews etc. Handle information which may be sensitive, complex, or confidential appropriately, in accordance with the Data Protection Act and local information governance policies. Be responsible for managing own workload and prioritising, as necessary. Undertake training and development, to ensure the required knowledge and skills for practice-based work.
