An exciting opportunity has arisen to join the 0-19 Service at Sheffield Children's NHS Foundation Trust as a 0-19 Specialist Vulnerabilities Practitioner.

The post holder will be a key member of the 0-19 Service, who provides Community Public Health delivery through the Healthy Child Programme, and Public Health Promotion. They will be over 2 Locality Teams (Rivelin to Sheaf/Hillsborough Upper Don), who will effectively support the delivery of the above to ensure that the children and young people in Sheffield reach their full potential.

The 0-19 Specialist Vulnerabilities Practitioner will:

Work with the Team Leaders to identify vulnerable children and their families and ensure provision of enhanced health intervention and care packages based on identified needs.

Provide safeguarding supervision to the 0-19 team members with the support of the Safeguarding Team (In line with the supervision policy).

Support the implementation of restorative and clinical supervision to the 0-19 Team members through various methods.

30 hours per week.

Main duties of the job

The 0-19 Specialist Vulnerabilities Practitioner will be responsible for:

Supporting the 0-19 Service Manager and the 0-19 team members to identify vulnerable children and their families and support the provision of appropriate health interventions and care packages.

Providing guidance and support to individual case management where safeguarding supervision is required.

Providing specialist assessments and relevant evidence based interventions to families within this care group in a variety of 0-19 settings.

Participating in development and delivery of packages of care and pathways relevant to the area and support training and implementation of packages of care by others in the team.

Job description

Job responsibilities

Work in close partnership with the Team Leader and Named Nurse and Safeguarding Team to support service delivery and developments in line with the local and national policy directives relating to safeguarding.

Ensure practice is in line with Working Together to Safeguard Children, and the local safeguarding board.

Support the Team Leaders with the day to day management and supervision of the 0-19 teams and support delivery of the 0-19 service specification.

Provide supervision and management support for the 0-19 team members including skill mix staff in conjunction with the Team Leaders.

Work with the Team Leaders and Safeguarding Team to establish effective team peer supervision relating to safeguarding and support models of shared team learning related to safeguarding case work.

Work with the Team Leaders to assess incoming work, to identify early and engage the most vulnerable, at risk families and ensure that the children and families identified as vulnerable receive the appropriate intensive support/packages of care.

Work closely with the Multi Agency Support Team and Early Years Services to ensure a strong and coordinated service/support for those children and families identified as vulnerable.

Manage records and clearly document all care plans, case reviews and outcomes to reflect casework with children and families.

Will require flexibility in managing a clinical and specialist role.

Qualifications

Essential

Registered with NMC as a Community Specialist Public Health Nurse School Nursing or Health Visiting Mentorship qualification Post Graduate qualification at Masters Level or equivalent knowledge and skills gained through a combination of in-depth experience post graduate or post registration study, research and/or courses

Safeguarding Supervision/Training Nurse prescribing

To undertake the duties of the post the successful candidate will have significant and recent post registration experience, of which a considerable portion must be as a qualified Specialist Community Public Health Nurse (School Nurse or Health visitor) Significant experience of working with children, young people and families, safeguarding and vulnerable groups. Experience of safeguarding children, involvement in complex child protection, child in need and children looked after. Willingness to develop skills and knowledge required to provide safeguarding supervision/Clinical Supervision. Experience in the planning and delivery of Public health interventions, projects and/or programmes and developing packages of care.

Experience of implementing new ways of working and/or taking a lead in practice development/innovation.

Knowledge

Essential

Experience of Leadership skills within a locality team. A sound knowledge of local and national Child Protection and Safeguarding guidelines and policies. An in depth knowledge and lead in complex child protection, child in need and looked after children living in the Team locality. Sound awareness of the complex issues involved when working across organisation and disciplines. Knowledge and understanding of principles and benefits of reflection and use of models and tools to enhance understanding of complex clinical practice and child protection supervision. Demonstrates a sound and robust understanding of all aspects of Trust and NMC record keeping processes and requirement. Understanding the importance of a customer focused service, equal opportunities, privacy dignity and all diversity issues.
