Job overview

An exciting opportunity has arisen for an enthusiastic RMN to contribute to the continued development of the Primary Care Mental Health Services in delivering care and support to patients with long term mental health needs in primary care.

 You will operate within the principles of recovery and be responsible for the case management of agreed individuals with a stable but long term mental health condition in conjunction with the Primary Care Team.

You will work alongside GP surgeries where you will liaise with GPs on managing patients with a wide range of presenting mental health conditions. You will carry out mental health assessments, brief interventions and help to improve the mental health awareness and delivery of Mental Health Care in Primary Care. You will also lead on specific projects identified as a priority by your local Integrated Care Board (ICB).

Main duties of the job

The Primary Care Mental health Nurse is a key practitioner within the multidisciplinary care team and will have continuing responsibility to assess, plan, coordinate, deliver and evaluate care, communicate findings, influence and drive change and promote health and best practice, patient safety and experience.

Carry out and coordinate comprehensive, systematic nursing assessments which take account of relevant mental, physical, social, cultural, psychological, spiritual, and environmental factors, in partnership with service user and others through interaction, observation and measurement.

Work collaboratively with other disciplines, service user and their carers to agree a holistic person centred care plan that addresses the needs identified through assessment.

Deliver and evaluate safe, person centred care in partnership with the service user and their carers to support recovery.

To work collaboratively with KMPT services, KCC, Gp's and other Primary care partners including voluntary and 3rd sector organisations to share expertise on the delivery of Mental health Care .
