Job summary

This is an exciting opportunity to work as part of the new expanded Liaison and Diversion Service. The posts will be based in Middlewich custody suite but with the expectation to be flexible and cover the custody suites based in Runcorn and Chester if post will increase the capacity of the existing L&D team and will be working closely with the current role will include providing high quality assessments for people who are in the criminal justice system. The service will be delivered to peoples who present with any vulnerability including but not limited to; mental health, physical health, learning disability, head injury, unemployment, housing, educational needs successful applicant must hold a professional successful applicants must be experienced in risk assessment, highly motivated and someone who is looking for a new challenge and have the skills to make a to the nature of the work this post will be subject to police between criminal justice settings in Cheshire may be required.

Main duties of the job

1. Working in a Health and Justice Service in a Cheshire custody suite responsible for offering high quality clinical service to patients and their Assess patient's individual care needs, agree, and refer on to appropriate Responsible for clinical and caseload supervision of other team members where Liaising with multiple criminal justice services and other external Identify, assess, and refer people of all ages with a wide range of vulnerabilities when they are in contact with the justice Responsible for participating in the setting of quality standards, including the auditing, monitoring, and reviewing of practice in line with current clinical guidance practice and Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/ Maintain an effective reporting system by observing and reporting verbally and in writing on service users' presentation and Utilise agreed outcome measures to review the effectiveness of process, as part of Liaison and Diversion Participate in the development of the service to ensure clinical governance processes are actively implemented at a team level.

Job responsibilities

This is an exciting opportunity to work as part of the newly expanded Liaison and Diversion Service. The post will be based in Middlewich custody post will increase the capacity of the existing L&D team and will be working closely with the current staff.

The post holder will require professional registration RMN/RGN/SW/AHPThe Post holder will be expected to conduct high quality assessments within a custody setting with people who are in contact with the criminal justice system. The service will be delivered to people who present with any vulnerability including but not limited to; mental health, physical health, learning disability, head injury, unemployment, housing, educational needs etc.

The post holder will be required to work autonomously within this role and be accountable for managing referrals; prioritising referrals and ensuring clinical and electronic record keeping standards are adhered too.

The successful applicants must be experienced in risk assessment, highly motivated and someone who is looking for a new challenge and have the skills to make a successful applicant would need to be skilled to work with people of different ethnicities and backgrounds and experience of working with both adults and children/youths is desirable.

Due to the nature of work, this post is subject to successful police vetting.

Person Specification

Qualifications & Training

Essential

Registered Mental Health Nurse / Learning Disability Nurse / Registered General Nurse on NMC Register. Registered OT, Speech and Language Therapist or Social Worker.

Experience

Essential

Evidence of extensive post registration experience with working with Clients experiencing mental health related difficulties. Experience of working with Clients who have difficulty in engaging with services and have complex health & social needs. Experience of assessing, planning, co-ordinating and managing people with vulnerabilities in a community setting.

Skills, Knowledge & Abilities

Essential

Knowledge of the Mental Health NICE Guidelines & Knowledge of NMC Code of Conduct. Understanding of the principles of the Care Programme Approach and Community Care Act. Knowledge of the wide range of statutory independent and third sector service provision for adults with mental ill health Skills of assessing, planning and evaluating care, for people with mental health related difficulties, utilising the principles of Recovery.
