If so Sandwell and West Birmingham NHS Trust are looking to recruit you in the role as a Strengthening Families Practitioner Specialist Community Public Health Nurse (Health Visitor)

This is a new and dynamic role for SWB Health Visiting service where you will support the development of integrated working, embed our joint Early Help strategy, through partnership working and targeted intervention and building therapeutic relationships with families for improved health outcomes. This is a demanding specialist role requiring high levels of professional skills knowledge of safeguarding practice and the ability to work as part of a team.

Main duties of the job

The main role for the Strengthening Families Health Visitor is to be the delegated named Health Visitor for children requiring support for universal plus and partnership plus Health Visiting services as well as contributing to the public health agenda.

Strengthening Families Health Visitors will be responsible for carrying out holistic health assessments, providing public health, health promotion and illness prevention for their specific caseload of families who have complex needs at the earliest possible time and embedding the local Early Help strategy.

Experience

Essential

Significant years' experience of working within the community as a Health Visitor Evidence of managing a complex caseload providing high quality evidenced based care and recording outcomes in partnership with families Leadership skill with the ability to work autonomously and part of a team. Experience of providing training and specialist advice & supervision Demonstrate understanding of risks associated with working with complex families, and multi-agency partners to implement Early Help and Safeguarding plans

Qualifications

Essential

Specialist Community Public Health Nurse (Health Visitor) Educated to Degree Level or equivalent Up to date safeguarding training

Desirable

Educated to Masters Level or equivalent Leadership Qualification
