We are looking for an enthusiastic, dynamic and experienced Paediatric nurse, who is passionate about paediatric diabetes and achieving optimum health outcomes for all children, young people and their families living with this long term condition.

The post holder will hold a high degree of autonomy and responsibility for assessing, developing and delivering care pathways for children in collaboration with the Consultant team for diabetes and the MDT.

This is Band 7 role; however, Band 6 applicants would be considered. There will be upgrade to band 7 on completion of diabetes module/course as defined in -house, following a period of consolidation and assessment of competence. This will be assessed by the Paediatric Diabetes Nurse Specialist and line manager (Matron for Children's Services)

Main duties of the job

The role involves providing consistent care to the patients & families on our caseload from admission at diagnosis, through clinic appointments, any subsequent admissions, to transition to adult services.

This will include community visits to homes and schools, structured education sessions, support for multidisciplinary clinics, support for patients with insulin pumps and their families such as education / training at pump initiation and monitoring its continued use. You will need to demonstrate you can be flexible and manage change in a positive and effective way. You must be prepared to work both in the community and in the acute setting.

Detailed job description and main responsibilities

The Paediatric Diabetes Nurse Specialist (PDNS) hold responsibility for the provision of highly specialist nursing for children and young people with diabetes (CYPD) from diagnosis through to transition within the area served by University Hospitals Sussex NHS Foundation Trust.

Practicing autonomously as a core member of the multi-disciplinary team they will accept referrals to agreed criteria and be the identified key worker for a patient caseload to co-ordinate treatment and care.

To act as a knowledgeable practitioner in paediatric diabetes supporting the families affected by diabetes, they will provide specialist advice, education and support to patients, their carers and other health care professionals in the home, hospital and school/nursery settings.

To Act as the advocate for CYPD, assessing, planning and implementing the most effective nursing care.

Supporting CYP and their carers in the acquisition of the necessary technology skills, supporting with insulin dose adjustment and empowering CYPD and their families / carers to self-manage.

To discuss specific patient care/management issues with nursing, medical staff and other members of the multi-disciplinary team (MDT).

To liaise with GP’s, health visitors, school nurses, social workers and child and family therapy (C&FT) on an individual patient basis.

To discuss sensitive condition related information (e.g., ongoing plans of care and current management) with patients and their families.

The post holder will actively undertake clinical audit for the paediatric diabetes service against national standards.
