Job summary

An exciting opportunity has arisen within the trustwide Tissue Viability Team. The post holder will be adynamic individual who will motivate health professionals to promote anddevelop nursing care in all areas of tissue viability; creating and maintaininga culture of effective patient and health care services. To promote pressureulcer prevention and reduce patient harm.

The post is multifaceted as it has clinical, educational,audit, research and development components. The post holder will have skillsand knowledge in the field of tissue viability, you will be required to workautonomously in performing assessments of wounds and implement wound managementand pressure ulcer prevention plans. You will provide advice and support tohealth care professionals within the Trust and across Mid Yorkshire Communityto improve care delivery and outcomes with support from senior Tissue Viabilitycolleagues.

Main duties of the job

The post holder will provide advice and intervention to allpatient groups with tissue viability needs, promote a seamless service within amulti-professional framework and will contribute to the development of thetissue viability service. You will be responsible for the provision of adviceand support to Trust and non-Trust healthcare professionals in relation toTissue Viability. Provide education and training on Tissue Viability, includingwound management, pressure ulcer prevention, pressure relieving equipment andleg ulcer management.

To discuss/demonstrate dressing choices/techniques forwounds at request of referring professional.

Liaise with acute Trust, community district nurses,residential homes, nursing homes, and other disciplines regarding themanagement, discharge and transfer of patients with Tissue Viabilityrequirements.

The post holder will contribute to achieving the objectivesof the Trust by implementing quality improvement strategies in relation topressure ulcer prevention to reduce trust acquired patient harm.

Take an active part in the development of nursing in theTrust via participation in professional nursing forum and shared governanceinitiatives.

Job responsibilities

To provide clinical, social and psychological support to their patients using excellent communication skills, education and health promotion to enable them to reach their optimum level of independence and health To provide advice, supervision and support to their defined caseload and peers. Responsible for providing evidence based practice Will have responsibility for supporting the Matron for Tissue Viability in leading identified and agreed aspects of the service, in the development of care systems, protocols and guidelines working as part of a multi-disciplinary team and developing excellent interdepartmental relationships with excellent communication with a focus on quality improvement Work collaboratively with the Tissue Viability Team and Allied Health Professionals, with in their own scope of practice Will ensure cost effective use of all resources Promote programmes of health education and health promotion relevant to service Work within national policy frameworks relevant to the service Deal effectively and professionally with patient / service user issues collaborating with PALS as appropriate and take a proactive approach to resolving issues and learning from complaints and escalate when unable to resolve Contribute to the multi-professional approach in the management of patients / carers within the specialist area in relation to ongoing care needs Act as a resource for tissue viability knowledge for clinical staff in the Care Closer to Home division. Ensure effective multi- disciplinary communication Support the management function of the department by provision of reports and information as required Person Specification

Registered General Nurse Adult part 1. First level degree or demonstrable experience in Tissue Viability. Evidence of continued professional development. Independent Nurse Prescribing qualification/willingness to undertake.

Recognised teaching qualification. Non-medical prescribing. Completion of Tissue Viability/Woundcare course
