Job summary

An exciting opportunity has arisen to join the Tissue Viability/ Leg Ulcer Service delivering care to the adult population of West Essex. This role will include working as part of the Integrated teams, in delivering wound care and treatment in the primary care and Nursing Home setting which includes leg ulcer clinic and domiciliary visits

The successful applicant will have extensive experience within wound care.

They will have admirable communication and listening skills and conduct themselves professionally at all times.

They will be dynamic in their approach and be able to demonstrate abilities to adopt a problem solving approach to supporting and implementing the management plans for these complex patients whilst working independently.

The successful applicant will demonstrate an ability to work well within a team and have an understanding of the structures of primary and secondary care services and how the two organisations interface, particularly within the Long Term Conditions arena.

Whether working in a clinic or domiciliary setting the team's vision is to deliver a sleek, modern evidence based service to all patients within settings close to or within their homes to promote independent self management of their chronic condition, consequently avoiding admissions.

The successful applicant will be supported to develop within their role underpinned with a robust appraisal, supervision and personal development plan to achieve their agreed objectives

Main duties of the job

The post holder will be responsible for providing the daily running of the Tissue Viability/Leg Ulcer Service in the delivery of a high quality clinical service within Private Nursing Homes and NHS Provider Units. The post holder will provide professional advice, support and education to the multi professional team, patients and carers based on NICE Guidelines, and best practice recommendations in relation to wound management strategies. The post holder will be proficient, knowledgeable and competent. They will continue to develop in their role through the acquisition of further skills, knowledge and competencies, to be determined by the needs of the patients, the service and the team. The post holder will work as an autonomous practitioner supporting the delivery of care and working in collaboration with system partners and as part of the Primary Care Networks and the emerging Integrated Neighbourhood Teams as part of the out of hospital model of care UK driving license and access to a vehicle - essential when applying for this post

Job responsibilities

Clinical & Professional Duties

1.Independently provides holistic, pressure ulcer risk and wound assessments for individuals with health impairment, facilitating the identification of adverse conditions that impair tissue health.

2 Plan, deliver and evaluate programmes of care for patients referred to the TVS in conjunction with referrer.

3 Creates and utilises opportunities to promote health and wellbeing of patients, empowering them to make informed choices and promote independence where appropriate.

4 Promotes clear, concise and specific documentation in relation to tissue viability issues.

5 Promotes good working relationships within the multi-professional teams providing appropriate specialist advice and support.

6 Working within the NMC Guidelines and Trust Policies recognising ones own abilities and limitations.

7 To act as an accountable role-model through the provision of professional leadership and demonstration of competent practice.

Patient/Client Care

1 Assess the patients referred to the TVS holistically, using a range of different assessment methods and reach valid, reliable and comprehensive patient centred conclusions, which manage risk and are appropriate to need, context, culture and ethnicity.

2 To provide specialist nurse advice, as requested, for patients with tissue viability problems in private nursing homes, clinics, health centres, patient homes, community hospitals and other settings.

3 To ensure that all advice provided is evidence based and in line with Trust, TVS and national policies and guidelines.

4 To follow all Trust guideline in relation to safeguarding vulnerable individuals.

Communication Skills

1 To maintain confidential patient records and follow policies with regard to patients confidentiality.

2 To be responsible for communicating effectively and clearly both verbally and in writing.

3 To deal with any unacceptable practices through the accepted channels.

4 Develop effective communication networks with other health professionals across primary care.

5 Proactively and directly imparts specialist knowledge and shares specialist skills.

Education and Research

1 To work within the Tissue Viability/Leg Ulcer Team in creating and promoting a supportive learning environment that embraces a culture of evidence and research based practice.

2 Participate with the development, co-ordination and delivery of education and training in a variety of settings to different disciplines and levels of staff to facilitate the implementation of best practice and promote and support high quality patient care.

3 Be an educational resource for the provision of expert nursing advice on appropriate use of wound dressing products/technologies, pressure therapy technologies and evidence based care interventions to facilitate the effective management of pressure ulcer risk and problematic wounds.

4 Accept personal responsibility for identifying own learning needs and professional development and demonstrate a commitment to develop skills practice and knowledge within the speciality.

5 Create a learning environment which actively facilitates learning.

6 Participate in the auditing of wound and pressure injury management as required.

7 To facilitate the implementation of clinical governance through the implementation of evidence/research based guidelines, education and audit.

8 To facilitate the dissemination of appropriate research findings to the clinicians within the Trust ensuring clinical nursing activities are evidence based.

9 Critically review research findings, which relate to Tissue Viability practice and where appropriate sharing this information in teaching sessions and seminars thereby promoting a culture of evidence-based practice.

Managerial Responsibilities

1 To work in a cost effective manner, demonstrating speciality knowledge and high standards of nursing care whilst working within available resources.

2 To carry out administrative duties as required.

3 Comply with all local policies and procedures laid down by the Trust.

4 To participate with the development and implementation of the Tissue Viability/Leg Ulcer Business Plan.

5 Demonstrates specialised decision making skills, working under the direction of and in consultation with the Tissue Viability/Leg Ulcer Nurse Specialists to identify, analyse, challenge and halt poor practice to facilitate improved standards of practice.

Education and Qualifications

Essential

Professional registered Experience in a Community Setting Completed Study Diploma/Degree Level Evidence of other post registration education and training relevant to Wound Care Evidence of personal development

Desirable

Teaching and assessing qualification Nurse Prescribing

Knowledge

Essential

Evidence of clinical knowledge/skills in Complex Wound Management TNP/Leg Ulcer Management Knowledge of services provided by other agencies. Knowledge of health promotion approach.
