Job summary
Carmel Medical Practice is looking for an experienced Pharmacy Technician to join our core responsibilities of a Pharmacy Technician willcover clinical, technical and administrative duties within the Practice workingunder the supervision of the Practice Pharmacist. The postholder will ensureeffective and efficient use of medicines. This will involve targeted medicationreviews/reconciliation, review and coding of clinic and hospital dischargeletters, supporting patients with their medicines, health promotion, supportadmin team in signposting for patient self-care.
The position is full time/part time/flexible which can be discussed at interview. Hybrid working will also be considered.
Interviews will take place week commencing 2nd September 2024.
Main duties of the job
Completing prescription requests
Processing/Coding diagnosis from clinic and dischargeletters
Helping with medication changes from clinic anddischarge letters
Working with the pharmacist running High Risk DrugMonitoring Audits and arranging appointments (DMARDs, DOACs, Lithium,Antipsychotics)
Keeping up to date on Medicines Safety Audits MHRAAlert Audits on Ardens and processing results
Prescription queries from patients
Medication synchronisation
Monitoring CD prescriptions andordering
Medication requests fromDNs/Hospices/Dieticians where appropriate
Helping towards QOF (ensuring appointments are made forlong term conditions in line with birth month etc)
Medication review monitoring etc ensuring urate isdone for Allopurinol, ECG for QOF, Prolactin for Antipsychotics etc
Updating Ophthalmology meds/drops
Medicines Optimisation projects in line with localdrives and formulary updates (deprescribing OTC meds, checking iron/folate onrepeats which levels are now normal and no longer needed).
Checking formulary items/updates making costeffective swaps
Issuing medication alternatives when out of stock
Helping change medication to liquids if swallowingdifficulties
About us
Carmel Medical Practice has almost 11,500 patients which we care for from our purpose-built premises in the West End of Darlington. Our team comprises of 4 GP Partners, 1 Salaried GP, 2 Nurse Practitioners, 1 Clinical Pharmacist, 4 Practice Nurses and 3 Healthcare Assistants. The practice is run but the Practice Manager with the assistance of an excellent reception and administrative team.

ClinicalResponsibilities:
Undertake patient facing and patient supporting rolesto ensure effective medicines use, through shared decision-making conversationswith patients.
Carry out medicines optimisation tasks includingeffective medicine administration ( checking inhaler technique), supportingmedication reviews, and medicines reconciliation.
Where required, utilise consultation skills to work inpartnership with patients to ensure they use their medicines effectively.
Support with medication reviews and medicinesreconciliation for new patients and synchronising medicines for patienttransfers between care settings and linking with local community pharmacists.
Provide specialist expertise, where competent, toaddress both the public health and social care needs of patients, includinglifestyle advice, service information, and help in tackling local healthinequalities.
Take a central role in the clinical aspects of sharedcare protocols and liaising with specialist pharmacists for more complexpatients.
Support initiatives for antimicrobial stewardship toreduce inappropriate antibiotic prescribing.
Assist in the delivery of medicines optimisation andmanagement incentive schemes and patient safety audits.
Supportthe implementation of national prescribing policies and guidance within GPpractices, and other relevant primary care settings. This will be achievedthrough undertaking clinical audits ( use of antibiotics), supportingquality improvement measures and contributing to the Quality and OutcomesFramework and enhanced services.
Technical and AdministrativeResponsibilities:
Work with the Practice Pharmacist/PCN Pharmacy Hub team to ensure efficient medicines and system optimisation.
Supervise practice reception teamsin sorting and streaming general prescription requests, helping to implementclinical monitoring during the reauthorization process.
Provide leadership for medicines optimisations systems.
Provide training and support onthe legal, safe and secure handling of medicines, including the implementationof the Electronic Prescription Service (EPS).
Developprofessional relationships with other pharmacy and healthcare professionals inthe area.
