Job summary
We are looking for a Nurse Practitioner to join our friendly and highly skilled nursing team at a particularly exciting time. Following a relatively recent merger between two practices you would be joining at the ideal time as we look to develop and optimise the skill mix of a nurse led team.
The role would be varied and provide the opportunity for personal development as you will be a key member of the nurse led same day team.
Based in Reydon, just outside the picturesque Suffolk seaside town of Southwold.

Main duties of the job

To provide high quality, responsive and empathetic nursing care to the patients of Sole Bay Health Centre, including:
running Same Day Team clinics for the management of acute problems, including triage, diagnosis and treating/prescribing as appropriate;
referrals to secondary care/other health professionals as appropriate;
management of long term conditions;
visit housebound patients as necessary to ensure they keep well and avoid acute admissions where possible.
To work flexibly and supportively as a key member of the nursing team and wider multidisciplinary practice team.
To assist with the management of the Nursing Department.
To participate in teaching other nursing staff, medical students and other students as appropriate.
To participate in Research within the Practice.

SPECIFIC RESPONSIBILITIES
CLINICAL
To provide a holistic, autonomous and clinical nursing service both as part of the Same Day Team and in pre-booked clinics. To make an assessment of patients physical, psychological and social needs, either in practice or in the patients home, initiating diagnostic procedures and treatment including appropriate prescribing and referral. To set standards of care by following agreed protocols. With the support of the GPs, manage patients with Long Term Conditions, referring to Consultant Physicians as appropriate. Prescribe safely and effectively at all times
PROFESSIONAL
To provide a high standard of professional conduct and nursing care at all times in accordance with the NMC. To participate in teaching other nursing staff, medical students and other students as appropriate. To participate in Research within the Practice. To attend relevant meetings (in-house and external), courses, seminars and keep professionally up to date. To ensure nursing activities remain within broad policy objectives of the practice and a role in developing policy objectives.
MANAGEMENT & ADMINISTRATION
To help develop and maintain Practice policies and protocols for all nursing activity. To assist in maintain any Infection Control procedures, policies and audits within the Practice and ensure all staff are aware of current procedures. To keep adequate records and statistics of procedures performed including details of all batch numbers, etc. Be conversant with all computer systems utilised by the practice including the use of templates to ensure all necessary data is recorded and use these for the benefit of patients, self and colleagues. To provide holiday/sickness/study relief for the Nursing Team. To assist with the management of the Nursing Department.
GENERAL RESPONSIBILITIES
It is important that all members of practice staff are flexible and prepared to adapt and take on additional duties or to relinquish existing duties in order to maintain efficient running of the practice. The Board of Partners reserves the right to reasonably redistribute duties and functions from time to time based on service needs and the existing skills of the post holder. The post holder will maintain up to date professional registration as a requirement of the role and will comply at all times with the Code of Professional Conduct, assessing own performance and taking accountability for own actions, either directly or under supervision. Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance Work effectively with individuals in other agencies to meet patients needs Effectively manage own time, workload and resources. The post-holder will attend relevant training as identified either by Personal Development Plan or to meet the requirements of the practice. It is expected that the knowledge and skills acquired from attendance at such training will be shared and applied within the practice. The post holder is expected to personally comply with the statutory provisions of health and safety at work legislation and local policy and to always work in a safe manner and not to put self or others at risk. QUALIFICATIONS Registered Nursing or Midwifery Qualification Degree / MSc modules in Advanced Clinical Skills / diagnostics Non-Medical Prescribing Qualification EXPERIENCE Minimum of 5 years post registration experience Evidence of working in a primary care setting ( GP practice / Walk-in centre / Urgent Care Centre) at a higher clinical level, as an autonomous practitioner.
