Main duties of the job

As part of a team of Clinical Pharmacists and technicians you will be responsible for providing safe and effective medicines optimisation services to agreed community hospital sites. In addition, you will provide pharmaceutical advice to a range of specialist services.

You will develop an expertise in the optimisation of medicines in frailty, working with multidisciplinary teams to ensure that we achieve the best medicines related outcomes for the patients we serve.

You will be surprised at the wide range of services and healthcare professionals and what links we have working with acute Trusts, GPs PCNs and multidisciplinary teams. Qualifications

Essential

MSc in Pharmacy (entry level for pharmacy). Registration with General Pharmaceutical Council.

Desirable

Postgraduate Clinical Diploma Independent Prescribing qualification

Experience

Essential

Post registration community or hospital experience Understanding of practical medicines management issues in community based services.

Desirable

Experience and knowledge of hospital practice including dispensing and clinical experience. Clinical experience of End of Life Care Experience of whole system working and collaboration

Knowledge

Essential

Knowledge of risk management and assurance issues relating to all aspects of medicines use. Familiar with the concepts of evidence-based medicine. Understanding of the concepts of Clinical Governance.

Desirable

Knowledge and experience of provision of pharmacy services to community services Clinical Knowledge of medicines in frailty.

Essential

Able to work independently and on own initiative.
