Main duties of the job

The post holder must be a registered pharmacist, whoacts within their professional boundaries, supporting and working alongside amultidisciplinary team in general practice.

The role will include patient facing servicesranging from telephone consultations to face to face consultation.

The post holder will take responsibility for areasof chronic disease management within the practice and undertake clinicalmedication reviews to proactively manage patients with complex polypharmacywhile addressing both public and social care needs of patients in the GPpractices.

The post holder will provide primary support togeneral practice staff with regards to prescription and medication queries, andwill provide support in the prescription hub, providing expert advice onmedicines, the repeat prescription system, manage acute prescription requests,and medicines reconciliation on transfer of care and systems for saferprescribing.

The post holder will provide clinical leadership onmedicines optimisation and quality improvement and manage some aspects of thequality and outcomes framework and enhanced services.

The post holder will ensure that the practiceintegrates with community and hospital pharmacy to help utilise skill mix,improve patient outcomes, ensure better access to healthcare and help manageworkload.

The role is pivotal to improving the quality of careand operational efficiencies so requires motivation and passion to deliverexcellent service within general practice.

Main Duties andResponsibilities

Patient facing long term condition clinics:

See (where appropriate) patients with single ormultiple medical problems where medicine optimisation is required (hypertension, asthma).

Review the on-going need for each medicine, a reviewof monitoring needs and an opportunity to support patients with their medicinestaking ensuring they get the best use of their medicines ( medicinesoptimisation). Make appropriate recommendations to GPs for medicineimprovement.

Patient facing clinicalmedication review:

Undertake clinical medication reviews with patientsand produce recommendations for senior clinical pharmacist, nurses and/or GP onprescribing and monitoring.

Patient facing carehome medication reviews:

Undertake structured medication reviews withpatients and produce recommendations for the senior clinical pharmacist,paramedics, nurses or GPs on prescribing and monitoring.

Work with care home staff to improve safety ofmedicines ordering and administration.

Patient facingdomiciliary clinical medication review:

Undertake structured medication reviews withpatients and produce recommendations for nurses and GPs on prescribing andmonitoring.

Attend and refer patients to multidisciplinary caseconferences.

Management ofcommon/minor/self-limiting ailments:

Managing caseload of patients withcommon/minor/self-limiting ailments while working within a scope of practiceand limits of competence.

Signposting to community pharmacy and referring toGPs or other healthcare professionals where appropriate.

Patient facingmedicines support:

Provide patient facing clinics for those withquestions, queries and concerns about their medicines in the practice.

Telephone medicinessupport:

Provide telephone clinics for patients withquestions, queries and concerns about their medicines.

Medicine information topractice staff and patients:

Answers relevant medicinerelated enquiriesfrom practice staff, other healthcare teams ( community pharmacy ordistrict nurses) and patients with queries about medicines.

Provides recommendations on medicine optimisationand adherence.

Offer follow up for patients to monitor the effectof any changes.

Unplanned hospitaladmissions:

Review the use of medicines commonly associated withunplanned hospital admissions and readmissions through audit and individualpatient reviews.

Put in place changes to reduce the prescribing ofthese medicines to highrisk patient groups.

Management of medicinesat discharge from hospital:

To reconcile medicines following discharge fromhospitals, intermediate care and into care homes, including identifying andrectifying unexplained changes and working with patients and communitypharmacists to ensure patients receive the medicines they need post discharge.

Set up and manage systems to ensure continuity ofmedicines supply to highrisk groups of patients ( those with medicine compliance aids orthose in care homes).

Signposting:

Ensure that patients are referred to the appropriatehealthcare professional for the appropriate level of care within an appropriatetimeframe, such as pathology results, common/minor ailments, acute conditions,long term condition reviews etc.

Repeat prescribing:

Implement the practice repeat prescribing policy.

Manage the repeat prescribing reauthorisationprocess by reviewing patient requests for repeat prescriptions and reviewingmedicines reaching review dates and flagging up those needing a review.

Ensure patients have appropriate monitoring tests inplace when required.

Risk stratification:

Identification of cohorts of patients at high riskof harm from medicines through pre-prepared practice computer searches

Medicines qualityimprovement:

Undertake clinical audits of prescribing in areasdirected by the GPs, feedback the results and implement changes in conjunctionwith the practice team.

Medicines safety:

Implement changes to medicines that result from MHRAalerts, product withdrawal and other local and national guidance.

Implementation of localand national guidelines and formulary recommendations:

Monitor practice prescribing against the localhealth economys RAG list and make recommendations to GPs for medicines thatshould be prescribed by hospital doctors (red drugs) or subject to shared care(amber drugs).

Assist practices in seeing and maintaining apractice formulary that is hosted on the practices computer system.

Auditing practices compliance against NICEtechnology assessment guidance.

Education and Training:

Provide education and training to primary healthcareteam on therapeutics and medicines optimisation.

Care QualityCommission:

Work with the general practice team to ensure thepractice is compliant with CQC standards where medicines are involved.

Public health:

To support public health campaigns.

To provide specialist knowledge on all public healthprogrammes available to the general public.

Qualifications/Required experience

Degree in Pharmacy, MPharm/BPharm

GPhC Registered

Full driving licence

IP Qualified or working toward an IP qualification

General Practice and understanding of medicinesoptimisation roles

Desired experience
