Job summary

Anexciting opportunity is available for enthusiastic, dynamic, and motivated Nutritionistswith experience and a special interest in weight management and healthpromotion. This role will contribute to thedevelopment, provision, and coordination of the Adult Weight Managementprogramme within the Healthy Communities Integrated Lifestyle Service foradults aged 18 and over and delivered across the county of Staffordshire.

We support allareas of Staffordshire including Tamworth and Lichfield, Newcastle and NorthStaffordshire, Stafford, Cannock, Burton, South Staffordshire, and surroundingareas. The successful candidate will be based in Burton-upon-Trent or Staffordbut will need to be open to travel and supporting teams across the area asrequired.

Thiscommunity-based, multi component service brings together a range of differentprogrammes under one Healthy Communities Service, centred on supportingresidents to lead healthier lives. The service offers a personalised carepackage for adults only, with a focus on those who need support to lose andmaintain a healthy weight, and via face to face or virtual offerings.

You will be responsible for providing high quality patient-centredcare and working within a multi-disciplinary specialist team. Providing expert nutrition advice,underpinned by a sound knowledge of the principles of behaviour change, youwill support adults across Staffordshire,to make and maintain health related behaviour changes.

Main duties of the job

The successful candidatewill be:

Passionateabout providing high quality care and support.

Able to deliver high standard nutritional care and advice withincommunity-based weightmanagement services.

Delivering one to one and group interventions, for patients who areobese, who may also have co-morbidities / complex needs.

Ideally have experience of delivering specialist care and treatment inhealth and community settings with a minimum 2 years experience.

Working as part of a multidisciplinary team and have currentregistration with the Association of Nutritionists (Anutr/RNutr) or BDA.

JOB DESCRIPTION

Nutritionist

MAIN PURPOSE

As anutritionist in our service,youwill deliver information to our service users about diet and nutrition, aimingto promote good health and disease prevention through behaviour changes to communitiesacross Staffordshire.Specifically, you will, in line with NICE guidance, be responsible andaccountable for the delivery of high quality, evidence-based nutrition advicefor adults seeking weight loss interventions in group settings.

SUMMARY OF RESPONSIBILITIES AND DUTIES

The Nutritionist will:

Provide comprehensive assessment of nutritionalneeds and patient lifestyle to develop care plans to address nutritionalissues.

Evaluate nutritional needs and on-going supportto patients in line with NICE guidance ina group and 1-2-1 settings, face to face and/or digitally, and implementingskills and knowledge of behaviour change theories.

Support the nutritional development, delivery, evaluation,and quality assurance of the integrated service and provide key nutritionalexpertise to the multidisciplinary team and other colleagues within theservice, ensuring excellent service delivery.

Facilitate and empower patients to changebehaviour and adopt positive lifestyle changes by providing health educationand high-quality nutritional counselling regarding healthy eating, helpingpatients to address barriers to change.

To attend multi professional meetings to providenutrition input regarding management for patients/clients including dischargeplanning and on-going management whilst respecting professional roles to ensurea supportive and proactive multi-disciplinary team.

Support community engagement, undertakenetworking publicity and promotional activity to raise the profile of theservice and to identify and engage with target populations.

Accountablefor ensuring programme delivery is monitored using validated tools to collectand analyse patient performance outcome measures and evaluation, to ensure thatthe contractual KPIs are achieved, and an excellent service is demonstrated; critical andrigorous implementation of all validated anthropometric measures at keymilestones across the services, in line with the Standard Evaluation Framework,ensuring excellent recording, monitoring, and reporting on patient outcomes.

Be responsiblefor ensuring rigorous and regular data inputting to ensure accurate records aremaintained in accordance with EH procedures, to ensure compliance withInformation Governance and Data Protection in relation to patient information.

Keepaccurate records and written reports, ensuring patient personal information isrecorded in accordance with Everyone Healths and Local Authority policies;Caldicott Guardian, Information Governance, Data Protection and Data Sharingcompliance.

Assist thedevelopment of continual service improvement and client/patient-centredservices in accordance with participant feedback, NICE guidance, national andlocal recommendations, professional competences, policies and procedures.

Be compliant with all relevant policies, procedures,and guidelines; including the appropriate code(s) of conduct associated withthis post; maintain awareness of safeguarding issues, report and acting onconcerns in accordance with Everyone Health and Local Authority policies andprocedures.

Ensure the safety of all patients byundertaking: risk assessments, health and safety checks, equipment checks etc.,as directed by the Service Manager and Clinical Lead. Respond to any relevant clinical / colleagueissues / incidences to manage a mitigate risk and maintain best practice.

Be responsible and accountable for achievement ofrelevant contractual targets including referrals, uptake, completion percentagetargets and effective outcome measures, through delivery of high standardnutritional care.

Activelyparticipate in community engagement to ensure services are deliveredefficiently and effectively, targeting priority groups to ensure thesuccessful recruitment and retention of patients within the service.

Take responsibility for own CPD and synthesisenew knowledge into the development of your own practice, participate intraining and continual professional development as required; recognise and workwithin own competence and professional code of conduct as regulated byprofessional body.

Ensure that commitment to equality and diversityis reflected at all stages of project planning, delivery, and evaluation, inline with agreed service standards.

Support in administrative duties related to yourpatients, for example letters to referrers, follow up phone calls, motivationaltext messages as required.

Be an ambassador for Everyone Health, promotingthe Healthy Communities Lifestyle Service to members of the public and otherstakeholders/ partners, where appropriate.

Soundknowledge of the complex care of the obese and morbidly obese patient.

Knowledgeof specific nutrition requirements for delivering specialist health improvementprogrammes for adults over 18 years of age.

4. QUALIFICATIONS

Relevantdegree/postgraduate degree in nutrition

Memberof the Association for Nutrition (AfN) or BDA

A minimum of2 years' experience in thisfield without registration (AfN/BDA) and working towards portfolio for AfNAccreditation (NutritionAssistant)

5. SPECIALCONDITIONS

Highly motivated self-starter with resilience,determination and the ability to support service users throughout the programme.

Good presentation of self: enthusiastic,innovative and flexible.

Role may incorporate evening / weekend working,with variable levels of travel across Staffordshire, for clinics andpromotional events.

Full UK Driving licence and access to a car forwork, with business purposes insurance.
