The applicant is afforded structured professional supervision on a regular basis, an annual appraisal and an extensive variety of learning opportunities.

This is a full time Community Nursing position which is designed to contribute to avoiding hospital admission and maintaining patient's independence in their own homes throughout the Gosport locality. This achieved by both routine visits but also responding to Urgent Crisis Response calls which are a key aspect of the role going forward. The UCR process is where as a team, we work alongside SCAS, offering acute visits with other members of the MDT, and using Community based skill set to try to reduce the need for an Ambulance to attend and to prevent the Patient being admitted.

This post offers the opportunity to work closely with all members of the ICT both in primary, secondary care and adult services as well as working closely with care home staff.

The job involves a proportionate amount of time driving therefore you will need to be a car owner and have access to a vehicle for work with the correct level of insurance.

Main duties of the job

Working within Professional guidelines and accountability, the post-holder will have responsibility for the management of a defined community caseload (via Nursing, Physiotherapy or Occupational Therapy interventions) for a group of GP Practices as part of the Integrated Care Team.

The post holder will have responsibility for managing co-ordinating and delegating work ensuring referrals are dealt with according to clinical need, Holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes.

The post holder will be required to work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community. The post holder will also have a responsibility for re-enablement, rehabilitation, health protection and promotion programmes that improve and reduce inequalities.
