Pharmacy Technicians play an essential role for the practices ofSouth Hardwick Primary Care Network (PCN) by complementing ClinicalPharmacists, Practice nurses, GPs, administrative practice teams, and othermembers of the PCN multi-disciplinary team.
ClinicalPharmacy in General Practice is part of an exciting programme of transformationto develop a new model of care which addresses our ambition to deliverperson-centred, coordinated care across the health system.
Pharmacy Technicianscan lead on medicines expertise and enable the Pharmacists to concentrate onclinical decision making. Core role responsibilities will cover clinical, technical and administrativecategories.
Main duties of the job
The purpose ofthe Pharmacy Technician role is to participate in the implementation ofstrategies to maximise achievement and monitoring of PCN national requirements,local guidelines, and national guidelines (NICE) implementation and utilisationof medicines optimisation initiatives.
The post holderwill help patients to get the best from their medicines by implementing agreedand approved protocols, around medications, including anticoagulation,cardiovascular medicines and inhalers.
In addition,the post holder must be willing to enrol and complete the Centre forPostgraduate Pharmacy Education (CPPE) pathway for PCN Pharmacy Technicians, orequivalent, if you have not already done so.

KeyResponsibilities:
Clinical
Undertakepatient facing and patient supporting roles to ensure effective medicines use,through shared decision-making conversations with patients. Carryout medicine's optimisation tasks including effective medicine administration( checking inhaler technique), supporting medication reviews and medicinesreconciliation. Whererequired, utilise consultation skills to work in partnership with patients toensure they use their medicines effectively. Asdetermined by the PCN, support medication reviews and medicines reconciliationfor new patients Synchronisingmedicines for patient transfers between care settings, linking with localcommunity pharmacies. Supportthe Clinical Pharmacist in Structured Medication Reviews (SMR) organisenecessary monitoring tests prior to SMR. Provideexpertise to address both, the public health and social care needs of patients,including lifestyle advice, service information, and help in tackling localhealth inequalities. Manageshared care protocols and liaise with Clinical Pharmacists for more complexpatients.Technicaland Administrative
Leadon clinical searches and monitoring of data. Participatein developing the PCN Pharmacy team in respect to developing policies andprocedures. Contributeto the design and delivery of patient questionnaires or pre-review resources torefine the system for medication reviews. Collateand filter information from the patient to be documented on the clinical systemor escalated to a Clinical pharmacist or other relevant clinician in thepractice, if appropriate. Supportthe practice Prescription's teams in streamlining general prescriptionrequests, to allow GPs and clinical pharmacists to review the more clinicallycomplex requests. Supportthe implementation of local and national prescribing policies and guidancewithin GP practices, care homes and other primary care settings. Supportthe Primary Care Network to deliver on contractual agenda, support Practiceswith QOF and locally commissioned enhanced services.General
