We have an opportunity for a qualified nurse (RN or RSCN or RMN or RLDN) to work with our multidisciplinary team made of paediatricians, nurses and support workers as we look to deliver a high-quality level of care.

We are seeking a highly skilled Clinical Nurse Specialist to join our team of nurses in the community paediatric service. This role involves assessing and supporting children/ young people with neurodevelopment disorders including assessment and management of children with ADHD prescribing and monitoring the effect of medication

The role is varied and you will be working with a highly supportive team made up of Clinical Nurse Specialists and PaediatriciansThe post holder will be a qualified nurse (RN or RSCN or RMN or RLDN) and will have extensive experience of working within a multi- agency team.

They will either have a Non-Medical Prescribing qualification or be willing to undertake this in the first year. The ideal candidate will have an additional post registration training Specialist Community Practitioner qualification - Health Visiting or School Nursing.

They will be an effective communicator with excellent interpersonal skills, displaying behaviours that are supportive and professional. This is a highly specialist post and all training will be provided for the right candidate.

Main duties of the job

The post holder will work within a multi-disciplinary team to contribute to the assessment, diagnosis, and support of children with neurodevelopmental disorders, including Attention Deficit Hyperactivity Disorder (ADHD) and Autism Spectrum Disorder (ASD). This will include undertaking medication reviews which involve independent prescribing.

The post holder will contribute to the redesign and development of services and pathways delivered from the Child Development Centre to meet the needs of the children.

Job responsibilities

Main Duties and Responsibilities:

Nature of Work. Provide specialist advice and nursing care in keeping with NICE guidance, and local guidelines around treatment options and age-appropriate care to optimise treatment. Contribute to transition planning with the adult services. Contribute to multi-professional assessments of neurodevelopmental disorders in children and young people referred to the Child Development Centre using up to date and evidence based assessment techniques and tools, assessing information from education providers and parents/carers alongside observation of the young person. This may include specialist assessments in various clinics across the trust, and observations in educational settings. Provide specialist nursing expertise within and across professional boundaries and develop and streamline Child/Young person care pathways between professionals/partners and in collaboration with other partners. Planning. Prioritise workload to ensure most effective use of time to meet current and changing needs of service. The post holder will plan their daily workload in accordance with agreed schedules. Liaise closely with other services in order to provide a co-ordinated and seamless service for Child/Young people and support the ongoing development of care pathways for these conditions. Project Involvement

Participate in specific projects as required both in the department and across the trust.

Patient Contact Where appropriate start, monitor and adjust pharmacological treatments and liaise with GP in line with agreed Shared Care Protocols Discuss and agree assessment outcomes and diagnosis with Child/Young person, carers and other health care professionals, promoting health education and self-management. Be able to advise and support parents and other staff ( school staff) in managing this group of childrens complex needs, including emotional and mental health issues; safeguarding / child in need issues and risk factors; multiple developmental difficulties and presentations and make appropriate and timely referrals, liaising with other professionals. Contribute to parent educational courses. Provide specialist nursing expertise within and across professional boundaries and develop and streamline Child/Young person care pathways between professionals/partners and in collaboration with other partners Be able to demonstrate a high level of clinical judgement & autonomous practice. This post requires the post holder to be an non medical prescriber and If you have a non-medical prescribing qualification, you will be expected to practice as a prescriber as part of this role Policy Development Work with others to Review current caseload, referral, assessment and review criteria and develop a nurse led service that provides an efficient and effective pathway Provide monthly activity data as required within the service specification. Participate as required in trust wide initiatives and service development activities. Contribute to the implementation of audits or research findings to improve services for Child/Young person. Contribute to the development and review of care pathways, clinical policies and guidelines. Prescribing Responsibilities

This post requires the post holder to be an non medical prescriber and If you have a non-medical prescribing qualification, you will be expected to practice as a prescriber as part of this role.

Communication

The post holder must demonstrate the ability to communicate with the Child/Young person of all ages and their families/ carers who may:

Have a varying degree of complex developmental difficulties, Neuro-disability or medical condition causing developmental disorder or delay ADHD/ASD. Be vulnerable as they are Children in Care Have complex needs where advanced communication skills are paramount. Be highly anxious or emotional due to news you are imparting. Be complaining about their care.

The Post holder will Work in partnership with Child/Young person and carers to enable them to make informed choices about care and treatment to promote their health and well-being

Ability to communicate with professionals both internal and external to Trust about a childs/ Young persons care. This will include writing reports.

Network and maintain effective working relationships with multi-disciplinary / multi-agency professions

Work in close collaboration with other community services within health, social services, and other voluntary and independent providers. Deliver training to Childrens Services staff and other identified partners as required on neurodevelopmental disorders in children. Provide education to parent groups and teaching staff in regard to neurodevelopmental disorders where appropriate, and facilitate therapeutic groups within the service for this group of children and young people. Budgetary Responsibility

Maintain identified equipment, in course of work duties including whilst in transit and also identify to line manager when equipment requires servicing and or replacement.

People Management To provide management supervision and line management where appropriate To assist and participate in the development of induction programmes for new staff to the team and associated teams within childrens services. To provide learning environment for students and colleagues by evidence-based practice. To participate in the staff appraisal process and to undertake for any staff you manage To keep yourself updated on all matters relating to Trust policy Research & Development Provide monthly activity data as required within the service specification. Contribute to the implementation of audits or research findings to improve services for Child/Young person. Contribute to the development and review of care pathways, clinical policies and guidelines. Involve Child/Young person, carers and families in clinical reviews and to be able to demonstrate service improvements in response to feedback received. Where necessary liaise with stakeholders Maintain a critical understanding of current practice Clinical and Practice Governance Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act. Any data that is taken/shared as part of a phone call or transported, faxed, or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies. The post holder must adhere to the Trust risk assessment and risk management processes. The post holder must adhere to infection control policies and procedures. It is a condition of your employment that you are currently registered with your professional body, and it is your responsibility to maintain your professional registration. This job role requires the clinician to undertake a non-medical prescribing function. It is the responsibility of the non-medical prescriber to:

- maintain their current registration with their professional body as a non-medical prescriber

- ensure they register with the Trust non-medical prescribing administrator

- fulfil all relevant continuing professional development requirements

- identify their own development needs in regard to prescribing

- work in accordance with Trust Medicines Policy (DN014) and relevant procedures

Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust. Provide patients and their families /carers with information on standards they should expect from the team. Participate in Clinical & Safeguarding supervision on a regular basis.
