Therole of the Nurse Practitioner will be to work in three blended specialities,Care Co-ordination (CCC) Single point of access (SPA) the Oncology helpline andTelephone triage.

Main duties of the job

Therole can be challenging, but is also very rewarding. You will require aholistic approach, with the ability to show both compassion and empathy, with acalm and genuine telephone manner Patients and their relatives or carers cancall for advice on any aspect of their condition or treatment, outcomes rangefrom advice and reassurance to emergency hospital admission. We areparticularly interested in hearing from Nurses with a background in oncology orhaematology, who understand the specific issues affecting patients undergoingtreatment in those specialties and the triggers for urgent hospital admission.

CCC

TheCare Co-ordination Centre (CCC) provides a unique telephone based service toprimary, urgent and emergency care colleagues, across Shropshire and Powys,helping to co-ordinate, advise and re-direct via relevant pathways, arrangingadmissions to emergency departments, hospital specialties and communityservices. Working closely with our partners in community care services torespond proactively, assisting with admission avoidance and therapeuticinterventions. We are particularly interested in hearing from Nurses with acommunity background who understand the pathways available for admission andadmission avoidance.

Oncology

TheOncology helpline is a dedicated, professional helpline with access to a teamof Nurses who are experts in this area. Following National Guidelines, endorsedby MacMillan Cancer Support and the UK Oncology Nursing Society (UKONS) theteam provide a 24-hour helpline for patients under the care of Oncology andHaematology services from several locations across England and Wales.

Telephone triage

Telephonetriage/CCC SPA is an in and out-of-hours service, triaging calls receiveddirectly from 111 and other OOH providers. Telephone triage here at Shropdoc isnot pathway lead and relies on the skill of the UCP to autonomously assessmultiple presentations and undifferentiated conditions forming a clinicaldiagnosis and treatment strategy, through the use of critical thinking andevidence based practice to support this.Early recognition of red flags is vital. Excellent verbal communicationand interpersonal skills are essential, as is multidisciplinary working, with apractical knowledge of a wide range of clinical conditions, with a clearrecognition of your limitations.
