You will havethe opportunity to join the discharge home to assess service. Discharge home toassess, assesses patient within the first 72 hours of discharge from hospitaland provide up to 21 days rehabilitation beds.

Support the Discharge Home to Assess Manager indelivering of high standard therapy service, including service evaluation.

Undertake therapeutic assessment of patients,some of whom may have complex and diverse conditions, and delegate tasks asappropriate.

Prior experience within the NHS HCPC registered OT/PT degree Experience of community working UK Driving License/Use of a car for work essential In addition to experience, all Qualified roles will be required to provide evidence of Relevant Qualifications Registration with the relevant Registration Body if applicable.
