Camrose, Gillies and Hackwood Partnership PCN works across 3 sites in Basingstoke providing high quality and personalised care for patients. The PCN locations are close to the M3 making then easily accessible from a variety of locations outside of the local area. This role is fundamental to the development and delivery of the primary mental health care offer. This offer is multidisciplinary and based within primary care. The person in this role provides clinical leadership and support to the day-to-day operation of the Primary Care Mental Health Team and support with development of the service. The team is made up of practitioners employed by the Primary Care Network, Southern Health and Mind. This role is supported by other clinical members of the team, as well as the PCN Manager and Clinical Director/GP Lead for Mental Health. The post holder will also provide direct clinical care within primary care. It will provide support to staff within the Primary Care Network and be engaged within the local community mental health team alongside all other partner agencies.

Main duties of the job

To provide clinical and managerial leadership to the day-to-day operation of the integrated clinical team, monitoring workload, allocating duties and ensuring duties are discharged safely and effectively.

To supervise the day-to-day work of the clinical team, and provide expertise and guidance in complex and high risk cases, in line with Trust and Local Authority policies and procedures.

To champion a culture based on principles of recovery, choice and personalised care, providing a role model to the rest of the team.

To take a proactive approach to caseload supervision and management to ensure that practitioner skills are matched to clinical need, and that service users are able to 'flow' through pathways of care.

To deputise for the Community Transformation Manager as required.

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
