JOBPURPOSE

As part of the Rotherham Healthwave team, ourHealth and Wellbeing Coaches support people from the local community to improvehow they manage their physical, and in turn their mental health. Through working with people both on a one toone or in a group setting, you will help people to develop the knowledge, skills,and confidence to change their health-related behaviours, helping to improve theiroverall quality of life, health, and wellbeing. These behaviours can include dietaryadvice, smoking cessation, and physical activity. As a Health and WellbeingCoach you will also be responsible for maintaining up to date and accurateclinical records. The role requires coaching skills, patience, and empathy tosupport people reaching their goals. A person-centredapproach is key to the success of the service, and although the role cansometimes be challenging it is also very rewarding.

Main duties of the job

1. Manage and maintain a caseload ofclients throughout their chosen lifestyle intervention.

2. Develop individual, evidence based tailoredsupport plans including recommending treatments based on assessed needs. Thiswill include individual and group-based interventions, signposting andreferring to other health, community, and voluntary services.

3. Work with clients to set SMART goals usingevidence-based knowledge, and NICE and NHS guidelines.

4. For clients that want to stopsmoking, you will be responsible for giving them advice on stop smokingmedications and vapes and will be responsible for ordering these medicationsand ensuring clients know how to use them correctly.

5. To contribute to the monitoring andevaluation of the service, including the completion of the required data set.This will include following up clients who do not engage with support andidentifying cohorts with multiple risk factors and most at risk.

6. Ensure that patient related documentationis entered into electronic health records with high degree of accuracy andcompleteness.

7. Work with managers and with ITsupport to maintain data quality and make improvements to data collection wherenecessary.

8. Assist managers and clinicians inachieving contracted targets by ensuring data is entered accurately and in a timelymanner.

About us

Connnnect Healthcare Rotherham Community Interest Company (CIC) is a federation of 30 GP practices in Rotherham. The organisation is led by a Board of representatives from general practice in the Borough, with executive leadership from a Medical Director on a part-time basis.

The organisation currently delivers extended access primary care services in Rotherham and a Physio First service in partnership with the local acute and community trust, the largest of its kind currently operating. In addition, the organisation is supporting the development of primary care networks and manages delivery of training and development to large cohorts of the general practice workforce.

Job responsibilities

The majority of our one to one andgroups sessions are delivered face to face at community venues, so thesuccessful candidate will be required to travel between venues in a timely manner.Assist with the induction process fornew staff.

To manageown time and priorities in providing service.

To undertake any other reasonabletasks as determined by managers and Connect team colleagues.

To ensure confidentiality andsecurity of patient data is always maintained in compliance with the Data ProtectionAct.

To ensure confidentiality of staff,patients and carers is maintained in line with organisations policy.

To support safe and effective teampractices.

To work closely with team members to providea flexible service and be willing to assist other staff as and when required.

Attend community and promotionalevents to talk about the service with health coaches as required.

To work as a member of a teamcommunicating effectively regarding work capacity and pressures ensuring thesmooth running of administrative provision. To raise with the line manager anybarriers to safe and effective working.

To attend all mandatory trainingcourses required and comply with health and safety regulations as part of personaldevelopment, , systems training, fire awareness training, PersonalDevelopment Reviews (PDR/Appraisals).

The successful candidate will be expected to commute from base to base around the Rotherham Borough area. Therefore, the candidate will need to have a full UK Driving License with access to a vehicle. THIS IS ESSENTIAL

Good knowledge of the Rotherham and surrounding areas

Further Training or Job Related Aptitude and Skills

Essential

Working knowledge of Microsoft Office. Ability to manage and prioritise own workload. Organisational skills Clinical record keeping. Attention to detail.

Desirable

Working knowledge of email systems Working knowledge of electronic health records systems, particularly using SystmOne

Knowledge Requirements

Essential

Computer literate, experience of using Microsoft Office software. Understand and maintain confidentiality. Ability to work and communicate in group settings. Excellent communication (written and verbal) and listening skills. Good organisational skills. Basic knowledge or experience of working in a healthcare environment.

Desirable

Knowledge of evidence-based smoking cessation initiatives. Knowledge of principles of health promotion and issues of health inequalities. Evidence of achieving targets and proactive approach. Experience of using clinical systems.

Qualifications

Essential

Educated to GCSE (or equivalent) grade C or above in English and Math. NVQ level 3 in Health care (or equivalent experience).

Desirable

Smoking cessation Level 2/3 NCSCT training (or willingness to train) Experience of working with groups (or willingness to train) Experience in health and lifestyle services with patients (or willingness to train)

Experience

Essential

Experience of working in the NHS in the community or hospital setting. Experience of providing healthcare, healthcare advice or health promotion advice to patients or members of the public Ability to deal with the public in an empathic and effective manner. Ability to liaise with all levels of professionals in a confident and effective manner. Experience of managing highly confidential and sensitive information.

Desirable

Experience of providing help, support and/or counselling on a one-to-one or group basis. Knowledge and experience of working with primary care-based health professionals. Knowledge of behaviour change models and experience of using motivational interviewing techniques.
