Job overview
The post holder will achieve this by working as an autonomous member of the Specialist School Nursing Team, supporting a defined caseload of children and young people aged 5-19 years with complex healthcare needs and disabilities within the community and school settings.
The post holder will carry out clinical procedures in partnership with other professionals and agencies in order to meet identified care needs.
The post holder will establish and maintain effective communication with children and young people aged 5-19 years, parents and carers, colleagues, special schools and the multidisciplinary teams in order to achieve a positive health status for the child and family.
The post holder will be a registered Children’s Nurse and be responsible for children and young people who are allocated to them and will assess, manage, plan and deliver care, including being professionally and legally accountable
Main duties of the job
1. To provide clinical practice leadership to the team members by acting as educator, facilitator, role model, mentor and preceptor.
2. To assist and facilitate a flexible team approach in meeting the health needs of the local school age population working with members of the team to identify areas of service development which meet identified needs and government.
To enable the team to develop and adapt in order to meet future health needs.
4. To encourage and facilitate the team to be reflective practitioners and to deliver effective research and evidence-based services.
5. To support the team to work towards achieving the service and clinical governance targets.

Detailed job description and main responsibilities
1.Work as part of the Special Schools and Complex Nursing Team, coordinating and delegating work as appropriate to the Community Nursery Nurses.
2.Identify and assess the health needs of school aged children and families in need (Health Care Plan’s). To work in partnership with Primary Care, Targeted Children’s Services, Education, Children’s Services, Voluntary Sector and any other appropriate agencies. To deliver integrated programmes of care, using the Common Assessment Framework where appropriate.
3.Work within Redbridge’s Safeguarding Children’s Board policies, prioritising attendance at Case Conferences, reviews and planning meetings.
4.Be responsible for the provision of case conference reports, Child in Need Reports and court reports as required, contributing to safeguarding of all children
5.Provide a continuum of support according to need, improving outcomes for children by reducing health inequalities, supporting the national strategy set out in the Children’s Plan (DCSF 2007)
6.Be the principal point of contact for a caseload of specialist schools within the area. To develop and deliver cooperative working arrangements with these schools that support healthy outcomes for children.
7.Promote and enhance health and wellbeing to children and their families through assessment, implementation of individualised health strategies and evaluation of care. (Choosing Health, Ch 1, Para 17, 2004)
8.Identify and assess children and young people aged 5-19 with complex health needs, offering an enhanced integrated service within the multi-agency arena, ‘enhancing equality and opportunity for them’, (Aiming High for Disabled Children, 2007)
9.The post holder will support their peers and team leader and be an effective team member whilst working on their own initiative. They will supervise pre-registration students and apprentice Nursing Associates. They will have a commitment to collaborative working and actively support and liaise with other health and social care professionals and agencies.
