Successfulcandidates will:
Haverelevant experience of working with autistic children and young people and/or alearning disability
Bepassionate about working co-productively and have experience of contributing topersonalised care plans
Havea good understanding of working in partnership and developing positiverelationships
Beassertive to appropriately challenge the systems and processes when they arenot working
Beinnovative with the ability to think out of the box using creative solutions toprevent furtherescalation, family or placement breakdown and admission into an inpatientsetting
Base/Location: One of the 4 NHS South Yorkshire Bases (Westgate Barnsley, SovereignHouse: Doncaster, Riverside House, Rotherham or Eyre Street, Sheffield), withregular travel across South Yorkshire and including hybrid arrangements workingfrom home.

Main duties of the job
The focus of the job role is to ensure the system is cohesively working together to ensure children, young people, carers and families receive the right care and support, from the right person at the right time including:
Providing emotional and behavioral support around complex needs, ensuring clear communication with all parties involved. Advocating in care discussions, maintaining accurate records of interventions. Building positive relationships with various professionals to prioritise home and community-based care. Troubleshooting and challenging practices to ensure appropriate and necessary support within the community. Providing evidence-based challenges to practices and decisions, identifying areas for improvement. Facilitating transitions into and out of inpatient settings, coordinating care across health, education, and social care. Communicating sensitive and complex information regarding their care in an empathic and supportive way.

Job responsibilities
As a Learning Disabilities and Autism Children &Young People Navigator, you will play a pivotal role in supportingchildren and young people (CYP) with complex needs and their families.
Yourprimary responsibility is ensuring the voice of CYP and their families arecaptured and heard and you will troubleshoot to ensure necessary support is inplace, ensuring that other professionals and services work cohesively aroundthe CYP to prevent further escalation, family or placement breakdown andadmission into an inpatient setting.
You willfacilitate emotional and behavioural support, promoting effectivecommunication, and foster positive relationships.
A key part of the role will be to navigate complex systems, challengethe status quo where required including any potential admissions to hospital toensure that all other options have been exhausted.
The role demands a keen understanding of health and social care andeducation systems, coupled with the ability to coordinate multi-agency effortsfor the benefit of CYP.

Ability to work on own initiative and organise own workload with minimal supervision working to tight and often changing timescales. Ability to measure outcomes based on agreed goals and to ensure this is fed into the overall evaluation of the services Demonstrable communication and influencing skills with the ability to remain calm, effective and efficient during difficult situations. Ability and willingness to handle confidential information without compromising confidentiality or trust. Able to deal with challenging people and to cope with confidential and sensitive information. Methodical approach and attention to detail. Advanced keyboard skills, use of a range of software.
Knowledge and Understanding
Essential
Experience of participating or understanding of Care Education and Treatment Reviews. Ability to work with complex cases and demonstrate empathy whilst gathering key information to inform agreed goals and outcomes for the C&YP. Demonstrating experience of dealing with a wide range of stakeholders and providing information and advice.
Desirable
Knowledge of Education, Health and Social Care sector. Previous experience in similar role in a healthcare or public sector environment.
