Job overview

The main purpose of the role is to triage all ‘same day’ requests that come into the Community Nursing Service via the Community Services Single Point of Access. The post holder will use their clinical skills to triage calls, referrals and messages, assessing and prioritising patient’s needs to ensure appropriate and timely care is delivered.

Main duties of the job

·Work as an autonomous practitioner providing a comprehensive telephone triage and clinical assessment service to patients, their carers/relatives, resulting in safe and appropriate clinical decision making

Upon completion of triage, ensure that any visits/ actions required, are referred onto and accepted, by the appropriate clinician/service. Ensuring appropriate escalation where onward referral cannot be confirmed

Provide clinical support to the SPA admin team, intercepting admin calls and speaking directly to callers where immediate triage is indicated or callers are demonstrating signs of distress or raising concerns

Participate in on-going audit and evaluation of the triage role promoting continuous improvement. This will include producing reports in relation to the triage role activity

Contribute to the improvement of services and the development, implementation and evaluation of protocols, policy and clinical guidelines

Maintain records of community nursing unplanned care requests. Undertake analysis and thematic review of this activity to identify trends and indicators to improve the quality of patient care and caseload management

Detailed job description and main responsibilities

These duties will vary slightly depending on where the post is within the planned or unplanned community teams, please refer to the triage process for the team this post is for.

You will mainly work in a non-clinical capacity, triaging messages against the referral criteria set out in the community nurse team SOP. All triaging will be completed via the electronic system, including the use of phone, utilising email, and the post holder will be required to liaise with the GP, health care professionals, patients and careers responsible for the referral. They will rotate into a clinical role to maintain their clinical skills.

You will work as a Triage nurse as part of the community nursing team to assess the referral/message against the integrated multi-disciplinary team specification to determine if the patent meets the criteria for IMT.

You will be responsible for working closely with the community nursing team to arrange schedules from the information provided via the triaging process for any patient referrals received.
