Job summary

We have an exciting new position available in our MK Adult Learning Disability, Mental Health and Autism Service. The Speech and Language Therapy Team is already well established within the integrated Social Care Adult Learning Disability Team in MK and also work alongside our social care colleagues in the Mental Health Team as well as those clients who have a diagnosis of Autism.

As part of your learning disability SLT role, you will working alongside Clinical Psychologists, LD Nurses, Occupational Therapists, Physiotherapists, Social Workers, Speech and Language Therapists and a range of Assistants who support the needs of this client group in the community. You will be working alongside the social care team when working with clients with mental health conditions and/or Autism.

You will join our Adult Learning Disability, Mental Health and Autism Speech and Language Therapy team under our wider Milton Keynes Speech and Language Therapy Service. There is a dedicated Speech and Language Therapy Team Leader, an experienced SLT assistant and full admin support. The team is based with other social care colleagues at Milton Keynes City Council Offices.

Main duties of the job

You will support clients to achieve what matters to them, to ensure they are involved in decisions about their lives through mental capacity work, provide support for all forms of AAC and support the delivery of training to various providers. The team work in a variety of settings: social care day services, clients homes, mental health settings and residential provider homes. In addition, the team work in post 16 specialist education provision to support relationship building as young people transition into adult services and MK College Further Education learning support dept. This involves working with students who have EHC plans and providing staff training.

The ALD service works closely together as a multidisciplinary team to ensure a holistic approach to both assessment and personalised support. The SLTs also support clients who have been admitted into hospital with dysphagia to provide continuity of care. Post- Graduate Dysphagia experience is preferable for this post but not essential as training and support can be provided to develop practice in this specialist field.

1.

MAIN PURPOSE OF JOB

Assessment, treatment and management of service users with Autism, mental health and learning disability who have speech, language and communication needs (SLCN) associated with these conditions in a range of settings.

Delivery of training to parents, carers, providers, employers and professionals on a range of topics related to SLCN

Assessment, treatment and management of dysphagia in service users with a learning disability (this is dependent upon experience)

PRINCIPAL DUTIES

To independently provide assessment, diagnosis and appropriate intervention for service users with speech, language and communication needs associated with Autism, learning disability and mental health conditions in a variety of settings.

To deliver training to professionals and parents/carers on a range of topics related to SLCN and dysphagia

To provide advice and support to parents, carers, non-specialists and other professionals working with adults in a range of settings including health and social care, education and employment settings.

To provide SEND support in specialist education settings for those young people and students who have EHC plans ( up to the age of 25 yrs)

To be an effective member of the SLT team and the broader multidisciplinary team

To participate in service development and support evidence for CQC KLOES

To undertake other duties, as requested by the line manager to ensure the efficient, effective running of the SLT Service

CLINICAL DUTIES

To assess, develop and implement speech and language therapy intervention

To provide advice to others regarding the management and care of service users

To participate in multi-disciplinary assessments

To ensure service users/parents/carers/providers/social care are involved in the planning and prioritisation of their care plans

To participate in multi-disciplinary planning of intervention/management to meet service users identified goals

To provide intervention and evaluate person-centred outcomes, and to demonstrate evidence based practice

To produce reports regarding service users needs and summarise proposed care plans eg EHC plans, positive behaviour support plans

To provide support/resources to the social care team so they are able to fully engage service users with Mental Capacity Act assessments, and work with social care team with regard to safeguarding, best interest meetings, Court of Protection and Mental Health Act.

To assess and manage dysphagia needs in service users with a learning disability

To adapt practice to meet individual service users circumstances, including due regard for cultural and linguistic differences (equality and diversity issues)

To demonstrate use of evidence-based practice

To seek second opinions on case management from senior colleagues
