Job summary

You will be a degree qualified Paramedic Practitioner with relevant clinical skills and expertise to work autonomously. You must have relevant experience enabling you to independently assess, diagnose, treat and refer patients as appropriate, as well as manage your own patient lists. Clinicians are available for guidance if required.

Main duties of the job

Theparamedic practitioner will be required to work autonomously in the practice, providing a range of servicessuch as assessment, diagnosis, treatment, telephone triage and referringpatients appropriately, as well as supporting the management team in thereviewing of clinical policy and procedure.

We adopt a team approach to patient care and endeavor to monitor the service provided to patients, to ensure that it meets current standards of excellence.

We occupy a purpose-built building with excellent road and rail links, and a nearby high-speed train service into St Pancras London.

We are currently using EMIS Web clinical system.

Job description

Job responsibilities

a.Identify,signpost or refer patients at risk of developing long-term conditions,preventing adverse effects on the patients health

b.Provideroutine care to patients as required in accordance with clinically basedevidence, NICE and the NSF

c.Examine,assess and diagnose patients and provide clinical care/management as required

d.Triagepatients and provide the necessary treatment during home visits

e.Triageand treat or refer patients who attend the surgery or over telephone

f.Collectpathology specimens as required

g.Maintainaccurate clinical records in line with extant legislation

h.Ensurethat read codes are used effectively

i.Ensurecontinuity of care, arranging follow-up consultations or reviews as necessary

j.Reviewthe effectiveness of the treatment provided, making changes where necessary toimprove patient outcomes

k.Chaperonepatients where necessary

l.Prioritisehealth issues and intervene appropriately

m.Actas the lead clinician when dealing with clinical emergencies

n.Recognise,assess and refer patients presenting with mental health needs

o.Supportpatients in the use of their prescribed medicines or over-the-counter medicines(within own scope of practice)

p.Liaisewith external services/agencies to ensure that the patient is supportedappropriately (vulnerable patients, etc.)

q.Ensurethat they adhere to the relevant patient group directives and local clinicalpathways at all times

r.Supportthe clinical team with all safeguarding matters, in accordance with local andnational policies

s.Understandpractice and local policies for substance abuse and addictive behaviour,referring patients appropriately

t.Deliveropportunistic health promotion where appropriate
