Job summary
An excellent opportunity has arisen that would suit a nurse with a hospital or community background to work as a member of the medway community anticoagulation service amongst a small and supportive team. As a valued team member, you will be responsible for clinical assessments of patients - You will undertake warfarin clinics in multiple settings within the community and patients' homes. Having a full driving licence and being a car owner is essential. You will contribute to influencing all aspects of care and management of patients within the specialty, and promote the delivery of high quality, clinically effective care, with education, point of care clinics, and remote telephone and software assisted dose titrations.
The service operates monday to friday 8am - 6pm, and saturday morning 8-1pm worked by a rotation. The service is responsible for nearly 2,000 patients across the medway towns and surrounding areas who are taking warfarin. Most of our patients are long term which means our staff get to know the patients well, and enjoy interacting with them.
Previous experience of working with an anticoagulation setting is not expected, as the successful candidate will begin by training within the service.
Main duties of the job
Accept written patient referrals ensuring that all patients registered with the service have an up to date record on the anticoagulation service database with a clear treatment plan documented. Participate in the computerised dosing of patients, liaising when required with nursing colleagues, line manager and medical staff. Deal with any clinical problems brought to your attention, and where appropriate bringing problems to the attention of the medical staff/nursing line manager. Provide a telephone and email advisory service to answer patients' queries relating to their anticoagulation. Participate in the new patient education clinic, providing or one-to-one patient education sessions for patients newly started on warfarin. Train and become competent with coaguchek point of care test devices, and participate in finger prick clinics, and in training patients who have purchased their own finger prick test device for INR monitoring. Participate in NEQAS external quality control scheme for near patient testing analysers, ensuring quarterly external quality controls are performed and reported to NEQAS Phlebotomy when requiredParticipate in regular review and rewriting of patient information leaflets, ensuring patient and peer involvement
