Job summary

Come and join us in the centre of the beautiful Cathedral City of Lichfield. We are a large supportive GMS practice (,000 patients and growing).

The successful candidate should be a Pharmacy Technician who is enthusiastic, highly motivated and will work part of a wider Clinical Pharmacy team. You be key to delivering high qualitypatient care, supporting medicine optimisation and pharmacy teams.

The Practice which is part of a wider Lichfield PCN Network team has excellent links with the local hospital, community pharmacies and carehomes in the area, so you would be truly part of a larger team working forbetter patient care.

Main duties of the job

To support the practice Pharmacist in delivering safe, efficient systems for the monitoring and management of repeat prescriptions, medicine optimisation and provide support and advice to patients to ensure the best possible outcomes and patient care achieved.

The Westgate Practice is a large GMS practice with 13 supportive Partners, located within the heart of Lichfield. We have a large team of c 100 staff working across a number of teams, including GPs, Advanced Nurse Practitioners, Practice Nurse, Pharmacy, Mental Health and administrative.

To respond to medication queries frompatients and staff in a professional manner, referring to the appropriateclinician in line with practice protocols. To support the Clinical Pharmacists inproviding medication reviews, developing further Standard Operating Procedures,Patient Information Leaflets and information about medicine awareness. To support the clinical pharmacists andadministrative staff in medicine reconciliation and updating of recordsaccording to practice and local policies. To liaise where necessary with communitypharmacists and other pharmacists in relation to medicine optimisation. To lead the practice in the auditing,monitoring and prescribing of high-risk drugs and assist in the delivery of medicines optimisation incentive schemes. (SLA) To take a proactive approach in the safehandling of prescriptions, assessing risks and making any recommendations forchange. To support the practice in achieving theQuality and Outcomes Framework and IFF targets and any targets from otherorganisations. To action medicines safety and MHRA alerts. To assist in the control of medicinesstocks within the practice, ensuring compliance with medicines legislation. To work alongside administrative staffinvolved in the preparation of repeat prescriptions on a daily basis to ensuretimely, safe high quality effective repeat prescribing. To help, plan, develop and support the introduction of new workingprocesses within the practice to optimise the quality of prescribing. Person Specification

Qualifications

Registered with the General Pharmaceutical Council as a Pharmacy Technician BTEC National Diploma in Pharmaceutical Science, NVQ level 3 Pharmacy Services or equivalent

Desirable

Qualified from an approved 18 month training pathway or equivalent (CPPE) Has experience and an awareness of common acute and long term conditions that are likely to be seen in general practice

Skills/Knowledge:

Essential

Excellent communications skills Good organisational skills Recognises priorities when problem-solving and identifies deviations from normal pattern . Able to follow legal, ethical, professional and organisational policies/procedures and codes of conduct. Involves patients in decisions about prescribed medicines and supporting adherence as per NICE guidelines. Demonstrate understanding of, and conforms to, relevant standards of practice. Demonstrates ability to identify and resolve risk management issues according to policy/protocol Follows professional and organisational policies/procedures relating to performance management. Demonstrate ability to extend boundaries of service delivery within the team.
