Job summary

This post offers an exciting opportunity for a clinical pharmacist to join our team as the senior falls pharmacist. You will deliver a high-quality, patient focused, clinical pharmacy service supporting the delivery of falls services across Bolton NHS Foundation Trust

The current pharmacy team works closely with the wider MDT forming a strong pharmacy presence across the integrated Community Division and pushing the boundaries of advanced practice. You will integrate as part of the MDT being one of the recognised experts on medicines in falls working closely with medical, nursing and AHP staff ensuring delivery of excellent care to people within Bolton

You will be supported by the Lead Clinical Pharmacist for the Integrated Community Services Division to further develop the role of pharmacy ensuring people receive the best possible outcomes from their medicines.

Main duties of the job

The Senior Clinical Pharmacist - Falls will work within multidisciplinary teams, providing highly specialised assessment advice and treatment to patients at high risk of falls within both the community and bed based services. The postholder will reduce the influence of medicines that may contribute to falls and other medicine related harms. They will supportevidence based prescribing and de-prescribing as appropriate

Job description

Job responsibilities

To work across the Integrated Community Services Division and acute trust To provide specialist advice on medicines related to falls and strategies to reduce the incidence of falls To provide clinical support as a senior Pharmacist to the wider pharmacy team. To visit patients in their own homes to offer advice and guidance on self-management strategies for individual patients. To provide specialist advice on medicines in falls to clinicians and AHPs regarding prescribing for individual patients and to support safe prescribing practice on an individual patient basis. To work across the division on agreed initiatives and joint projects as required and promote awareness of medicines management issues that may contribute to falls and other medicine related harms. Act as a deputy to the Lead Clinical Pharmacist in professional activities associated with the pharmacy service with regard to Medicine division/specialties activity and audit To conduct appropriate medication review for patients, communicating directly with patients as required. To support the development and implementation of prescribing policy and protocol within shared care settings such as Intermediate Care, Integrated Neighbourhood teams and bed based services To liaise with the Clinical Lead for Falls on in developing evidence based protocols and policy in relation to medication optimisation for those at risk of falls To liaise with the Medicines Optimisation team to meet local ICB objectives. To participate in relevant local, regional and national networks ensuring the communication of information and shared learning of patient safety incidents Where qualified to act as a non-medical prescriber in accordance with Trust policy and professional, regulatory and national guidance Person Specification

Vocational Master's Degree in Pharmacy Membership of The General Pharmaceutical Council Post graduate diploma in clinical pharmacy. A demonstrable experience of audit work and report writing

Evidence of leadership development Working towards or Registered Pharmacist Independent Prescriber Teaching/training experience Experience of appraisal and objective setting
